Abstract:

Práce je zaměřena na posouzení kvality výkonu mozku dle Rankinova skóre (Modified Rankin Scale, mRS) u pacientů po ischemické CMP v době propuštění z hospitalizace. Diplomová práce se snaží zhodnotit neurologický deficit po ischemickém iktu a faktory s ním související. Teoretická část obsahuje základní informace o charakteristice CMP, rizikových faktorech, managementu přednemocniční a nemocniční péče, vyšetřovacích metodách, rehabilitaci a prevenci. Praktická část byla provedena formou retrospektivního výzkumu. Zahrnuje analýzu a interpretaci získaných dat. Sledovaný soubor zahrnoval pacienty ve věku 21-98 let Práce se zabývá stupněm následků CMP hodnocených mRS. Průměrná hodnota modifikovaného Rankinova skóre v době propuštění z hospitalizace u sledované skupiny nemocných byla 2,15. V práci je hodnocen vliv věku na hodnocení funkční nezávislosti. Nejlepšího výsledek byl zjištěn ve věkové skupině 18 ? 35 let, kdy průměrná hodnota mRS činila 0,70. Průměrná délka hospitalizace činila 8 dní Práce se zabývá časovou prodlevou od vzniku symptomů do vyšetření neurologem. V časovém okně 4,5 hodiny bylo vyšetřeno 61,45 % nemocných. Práce se zaměřuje se na začátek léčby iktu. Do 3 hodin byla terapie započata u 33 % nemocných, v časovém rozmezí 3 ? 6 hodin u 24 % nemocných, v intervalu nad 6 hodin u 43 % nemocných. V práci je hodnoceno výsledné mRS ve vztahu k postiženému tepennému povodí. Příznivého výsledku mRS 0-2 dosáhlo 71,88 % pacientů po uzávěru ve vertebrobazilárním povodí a 56,88 % pacientů po uzávěru v karotickém povodí. Práce se rovněž zabývá výskytem rizikových faktorů u sledovaného souboru pacientů. Nejčastěji se vyskytovaly hypertenze a fibrilace síní. Při zkoumání vztahu rizikových faktorů a funkčního deficitu byl potvrzen vliv fibrilace síní na výslednou hodnotu mRS, a naopak se nepotvrdila souvislost mRS a diabetu. V závěru je kladen důraz na nutnost včasného přijetí pacientů na specializovaná pracoviště a potřebu vzdělávání laické i zdravotnické veřejnosti v dané problematice.

Abstract:

The thesis is focused on the quality assessment of brain performance according to modified Rankin scale (mRS) in patients after ischemic stroke at the time of discharge from the hospital. Evaluation of neurological deficit after Ischemic stroke and related factors ate the objectives of the thesis. Theoretical part contains basic information about the characteristic of stroke, risk factors, management of pre-hospital and hospital care, diagnostic methods, rehabilitation and prevention. The practical part was performed by retrospective research, includes analysis and data interpretation. Monitored data set included patients aged 21-98 years. This work deals with consequences of stroke assessed mRS degree. The average value of mRS at discharge form hospitalization for study group patients was 2.15. In this work was performed the influence of age on the performance evaluation of independence. The best result was found in the age group 18-35 years, when the average value of the mRS was 0.70. The average length of hospital stay was 8 days. The work deals with the time delay from onset of symptoms to a neurologist examination. In the time window 4.5 hours was 61.45% of the patients examined. The work is focused on the initiation of the therapy stroke. Therapy in 33% of patients was started within three hours, in 24% of patients at intervals of 3-6 hours, in 43% of patients with an interval of 6 hours. The aim of the study was to assess the resulting mRS also in relation to the affected arterial channel. Favorable results mRS 0-2 was achieved in 71.88% of patients after the occlusion in vertebrobasilar basin and 56.88% of patients after the occlusion in the carotid basin. The work also deals with the incidence of risk factors in the investigated group of patients. The most commonly was occurring hypertension and atrial fibrillation. In examining the relationship of the risk factors and functional deficit was confirmed atrial fibrillation influence on the final value of mRS and vice versa, confirmed continuity of mRS and diabetes. In conclusion, the emphasis is on the need for early admission of patients to specialized centers and the need for public and general education in health in this area.
