Abstract:

Práce přispívá k lepšímu náhledu na komplexní fenomén morfologických změn mozečku u pacientů s epilepsií temporálního laloku. Mozeček je komplexní struktura složená z anatomicky a funkčně rozdílných oblastí. Narozdíl od většiny předchozích studií zabývajících se mozečkem jako celkem zkoumáme přítomnost a charakter morfologických změn mozečkových substruktur. Analyzujeme vztah morfologických změn mozečku ke klinickým charakteristikám pacientů a průběhu epilepsie temporálního laloku. Poznatky získané detailní analýzou morfologických změn mozečkových substruktur nasvědčují význačnému postavení vermis v problematice epilepsie temporálního laloku s hipokampální sklerózou. Naše zjištění podporují domněnku o prognostickém významu atrofie mozečku z hlediska výsledku epileptochirurgického výkonu, řeší dřívější kontroverzi v této oblasti. Práce dokumentuje rozdílné morfologické projevy epilepsie pravého a levého temporálního laloku, navazuje na poznatky jiných autorů. Existence signifikantní strukturální kovariance mezi předním vermis a pravostrannými mesiotemporálními strukturami ve skupině pacientů s epilepsií levého temporálního laloku je dalším dílkem v mozaice studia funkčních vazeb v rámci cerebrocerebelárního konektomu, ještě více podporuje předpoklad o výlučnosti vermis v kontextu epilepsie temporálního laloku a nasvědčuje existenci funkčních vazeb mezi amygdalohipokampálním komplexem a vermis.

Abstract:

Our research contributes to better insight into the complex phenomenon of morphological changes of the cerebellum in temporal lobe epilepsy. The cerebellum is a complex structure composed of anatomically and functionally distinct areas. Unlike most previously published studies dealing with the cerebellum as a whole, we examine whether and how morphological changes differ within its substructures. We focus on the relationship of the cerebellar morphological changes to the characteristics of the patients and the temporal lobe epilepsy course. Exploration of morphological changes of the cerebellum in greater detail suggests an exceptional position of the vermis in the temporal lobe epilepsy with hippocampal sclerosis. Our research provides a dissolution of the previous contradictions regarding the prognostic importance of the cerebellar atrophy concerning the epilepsy surgery outcome. It supports the former observations of the different nature of morphological changes in left and right TLE patients. The existence of the significant structural covariance between the frontal vermis and right mesial temporal structures in the left TLE-HS group is another piece in the mosaic of the study of functional interconnections within the cerebrocerebellar connectome; supporting the significant position of the vermis in the context of temporal lobe epilepsy and the existence of a functional connection between amygdalohippocampal structures and vermis.
