Abstract:

BARTŮŠKOVÁ, Adéla. Komplexní ošetřovatelská péče o pacienta po cholecystektomii. Vysoká škola zdravotnická, o. p. s. Stupeň klasifikace: Bakalář (Bc.). Vedoucí práce: PhDr. Marcela Rybová. Praha. 2021. 75 s. Tématem bakalářské práce je komplexní ošetřovatelská péče u pacienta po cholecystektomii při respektování jejich přirozených potřeb. Teoretická část práce charakterizuje fyziologické funkce žlučníku a jeho onemocnění. Jako jsou žlučové kameny, zánětlivá onemocnění žlučníku a žlučových cest. Popisuje patofyziologii, symptomatologii, diagnostiku a léčbu onemocnění žlučníku. Práce se zaměřuje především na chirurgickou léčbu, a to cholecystektomii neboli odstranění žlučníku. Neméně významné jsou i pooperační komplikace, které jsou zde také definovány. Praktická část je věnována komplexní ošetřovatelské péči o pacienta, kterému byla indikována cholecystektomie. Tato část zahrnuje důvod indikace k odstranění žlučníku, příprava pacienta na operaci a pooperační péče na standardním chirurgickém oddělení. Důležitou součástí je i příprava pacienta k propuštění do domácí péče. Péče je koncipována do ošetřovatelského procesu, kde je posouzen celkový stav pacienta, stanovené ošetřovatelské diagnózy, cíle, intervence a jejich realizace. Ošetřovatelský proces byl následně vyhodnocen. V závěru praktické části jsou navržena doporučení pro praxi.

The theme of this bachelor’s thesis is the complex care of a patient following a cholecystectomy and the respect of their body’s natural needs. The theoretical portion of this thesis characterises the physiological functions of the gallbladder and the diseases that can affect its functionality, for example, gallstones, inflammation of the gallbladder (cholecystitis) and of the bile duct system (cholangitis). This section describes pathophysiology, symptomatology, diagnoses and treatments of the gallbladder. This work principally highlights a surgical treatment called cholecystectomy or gallbladder removal, without neglecting the complications that can arise post-surgery, which are also defined and covered with appropriate consideration. The practical portion is dedicated to the complex nursing care necessary for patients who have underwent a cholecystectomy. This section includes the reasons in which a gallbladder removal is recommended, preparation required for the patient pre-surgery, postoperative intermediate care and standard surgical ward care following the surgery. Another important aspect considered is the patient’s care plan for discharge. The care is assessed within the nursing process where the patient’s health conditions are being evaluated, assessed into nursing diagnoses, aims, interventions and realisations. Subsequently, the nursing process has been evaluated and the practical portion concludes with proposed recommendations for medical practice.
