Physiotherapy in painful menstruation

Abstract:

Souhrn Cíl práce: Přispět k řešení problematiky fyzioterapie u bolestivé menstruace. Metodika: U pacientky s diagnosou N 94. Bolest a jiné stavy sdružené s ženskými pohlavními orgány a menstruačním cyklem jsem s využitím technik manuální medicíny zhodnotila aktuální stav držení těla, měkkých tkání a stav pánevního dna. Vyšetřila pánev, sakroiliakální kloub a bederní páteř pomocí speciálních testů, provedla jsem testy na hluboký stabilizační systém páteře. Využitím vizuální analogové škály bolesti jsem zjistila subjektivní bolestivost menstruace. Na základě výsledků vyšetření jsem stanovila krátkodobý fyzioterapeutický plán, který byl realizován po dobu 14 týdnů, zpravidla 1x týdně po dobu 45 minut. Výsledky: Anteverze pánve, šikmá pánev pravá SIAS i SIPS výš, nestejná výše gluteálních rýh pravá níž, prohloubená bederní lordóza, protrakce ramen, hypertonus pánevního dna vpravo, blokády žeber, snížení rozsahu pohybu při extenzi pravé kyčle zkrácený m. iliopsoas, svalové dysbalance, varózní postavení levého kolenního kloubu a obou kotníků, bolestivá menstruace dle vizuální analogové škály bolesti 9/8. Na podkladě výsledků vyšetření aplikace průběžné terapie: techniky měkkých tkání, míčkování, postizometrická relaxace, mobilizace kostrče per rectum, mobilizace žeber a páteře, cvičení metody L. Mojžíšové a konceptu dynamické neuromuskulární stabilizace se zapojením senzomotorické stimulace. V průběhu terapie zmírnění anteverze pánve a s tím normalizace bederní lordózy, pánevní dno bez hypertonu, bez blokád žeber, zlepšení rozsahu pohybu pravé kyčle, posílení svalů hlubokého stabilizačního systému, snížení bolestí při menstruaci dle VAS 3/2-. Závěr: Vlivem intenzivní fyzioterapeutické intervence a vlivem edukace k autoterapii v domácím prostředí bylo dosaženo snížení bolestí při menstruaci o 6/6 stupňů na vizuální analogové škále bolesti, vyrovnání šikmého postavení pánve, zmírnění anteverze pánve, posílení gluteálních svalů, zvětšení rozsahu pohybu pravé kyčle do extenze. Ačkoli nedošlo k úplnému vymizení bolestivé menstruace a všech patologických postavení těla např. varózní postavení dolních končetin, výsledky nasvědčují o prospěšnosti fyzioterapie i autoterapie.

Abstract:

Summary Objective: To contribute to solving the problems of physiotherapy for painful menstruation. Methods: In a female patient with diagnosis N 94. Pain and other conditions associated with female genital organs and menstrual cycle, I used manual medicine techniques assess the current state of posture, soft tissues and condition of pelvic floor. Investigate pelvis, sacroiliacal joint and lumbal spine was examined using special tests and tests for core. Using a visual analogue scale of pain, I found a subjective painful menstruation. The short-term physiotherapy plan is determined based on the results of the tests. Plan was implemented for 14 weeks, usually once a week for 45 minutes. Results: Anteversion of pelvis, oblique pelvis the right SIAS and SIPS is higher, unequal amount of gluteal folds the right one is lower, deepened lumbar lordosis, protraction of arms, hypertonus of pelvic floor on the right side, blockades of ribs, reduced range of motion in the extension of the right hip - shortened m. iliopsoas, muscle imbalance, varus position of left knee joint and both ankles, painful menstruation on visual analogue scale of pain 9/8. On the basis of examination results, continuous therapy is applied: soft tissue techniques, balling, post-isometric relaxation, coccyx rectal mobilization, mobilization of ribs and spine, exercises with the method of L. Mojžíšová, and with the concept dynamic neuromuscular stabilization, involving sensorimotor stimulation. During therapy pelvic anteversion is alleviated and with that lumbar lordosis is normalized, pelvic floor is without hypertonus and without blockages of ribs, ROM motion right hip is improved, core muscles are strenghtened, pain during menstruation on visual analogue scale of pain 3/2- is reduced. Conclusion: With the effect of intensive physiotherapy intervention and of education on self-therapy in home environment, pain during menstruation has been reduced by 6/6 degrees on visual analogue scale of pain, oblique pelvis has been leveled, pelvic anteversion alleviated, the gluteal muscles strenghtened, range of motion right hip increased into extension. Although pains during menstruation have not disappeared completely together with pathological posture, e.g. varus posture of lower limbs, the results suggest the benefits of physiotherapy and self-therapy.
