Nutritional status of the elderly hospitalized in the internal department

Abstract:

Úvod: Malnutrice se projevuje jako nerovnováha (deficit/přebytek) energie, proteinů a dalších složek výživy, což má nežádoucí dopad na tkáně a celkový klinický stav (Jurášková 2007). S malnutricí se v nemocniční péči není těžké setkat. Proto je nutné vědět, jak rozpoznat pacienta v riziku podvýživy/v podvýživě, zvláště pak, pokud se jedná o pacienta seniorského věku. V seniorském věku se podvýživa vyskytuje četněji, kvůli fyziologickým a bio - psychickým změnám organismu. Pro správné hodnocení malnutrice je nutné znát screeningové metody, díky kterým je možné tento problém odhalit. Cíl práce: Cílem práce bylo vyhodnotit nutriční stav hospitalizovaných pacientů pomocí screeningových metod MNA (Mini Nutritional Assessment) a MUST (Malnutrition Universal Screening Tool) a následně jejich výsledky porovnat. Metodika: Průzkumné šetření probíhalo na interním oddělení. Zde byl prováděn nutriční screening pomocí MNA (Mini Nutritional Assessmet) a MUST (Malnutrition Universal Screening Tool). Testy byly prováděny současně, aby bylo možné porovnat výsledky. Nutriční screening byl proveden vždy první den hospilizace u všech přijatých pacientů nad 60 let (senior). Průzkum probíhal 6 týdnů za pomoci všeobecných sester a po písemném souhlasu hlavní sestry. Vyšetřeno bylo celkem 54 pacientů. Zjištění byla poté porovnána a interpretována pomocí grafů. Výsledky: Podle screeningového šetření MNA bylo zachyceno 53,7% pacientů v riziku malnutrice a 11,1% pacientů bylo vyhodnoceno jako malnutriční. Dle MUST testu bylo zachyceno 3,7% pacientů nacházejících se ve středním riziku malnutrice. Závěr: Podle zjištěných výsledků zachytil MNA screening na interním oddělení, více pacientů, kteří jsou v riziku vzniku malnutrice a již probíhající podvýživu než MUST dotazník. Dále také bylo zjištěno, že na tomto konkrétním interním oddělení, je více než dvě třetiny pacientů ve věku nad 60 let a tudíž jsou malnutricí více ohroženi.

Abstract:

Introduction: Malnutrition manifests itself as an imbalance (deficit/excess) of energy, proteins and other components of nutrition, which has an adverse effect on tissues and the overall clinical condition (Jurášková 2007). Malnutrition is not difficult to encounter in hospital care. Therefore, it is necessary to know how to identify a patient at risk of malnutrition / in malnutrition, especially if it is a senior patient. In old age, malnutrition occurs more frequently due to physiological and bio - psychological changes in the organism. For proper evaluation of malnutrition, it is necessary to know the screening methods, thanks to which it is possible to detect this problem. Aim of the work: The aim of the work was to evaluate the nutritional status of hospitalized patients using screening methods MNA (Mini Nutritional Assessment) and MUST (Malnutrition Universal Screening Tool) and then compare their results. Methodology: The exploratory survey took place at the internal department. MNA (Mini Nutritional Assessment) and MUST (Malnutrition Universal Screening Tool) were performed here. The tests were performed at the same time in order to compare the results. Nutritional screening was always performed on the first day of hospitalization in all admitted patients over 60 years of age (senior). The survey was conducted for 6 weeks with the help of general nurses and with the written consent of the head nurse. A total of 54 patients were examined. The findings were then compared and interpreted using graphs. Results: According to the MNA screening survey, more than 53,7% patients were at risk of malnutrition and 11,1% of the patients were evaluated as malnutrition. According to the MUST test, only 3,7% of patients at moderate risk of malnutrition were detected. Conclusion: According to the results, MNA screening in the internal department captured more patients who are at risk of malnutrition and ongoing malnutrition than the MUST questionnaire. Furthermore, it was also found that in this particular internal medicine department, more than two thirds of patients are over the age of 60 and are therefore more at risk of malnutrition.
