Abstract:

Úvod: Akutní pankreatitida (AP) je závažné onemocnění se značnou mortalitou i morbiditou a celosvetově vzrůstající incidencí. Univerzální a jasně ukotvená klasifikace je základem jak pro komunikaci mezi kliniky, tak zejména ve srovnávacích studiích klinického výzkumu. Porozumění epidemiologické charakteristice onemocnění a objev spolehlivých ukazatelů tíže onemocnění napomůže v nalézání efektivnějších postupů v rozhodovacím procesu i terapii pacientů. Metody: Analyzovali jsme 159 pacientů s AP hospitalizovaných v terciárním nemocničním centru, které jsme současně zařadili do kategorii původní i revidované atlantské klasifikace. Pomocí výstupových parametrů jsme hodnotili odůvodněnost zařazení do jednotlivých skupin tíže. Ohodnotili jsme BMI, počet bílých krvinek a RDW a jejich předpovědní schopnost ve stanovení tíže a úmrtnosti AP. Na souboru 261 postupně zařazených pacientů s AP z několika center Jižní Moravy jsme hodnotili incidenci, etiologické rozložení, podíl úmrtí a terapii. Výsledky: Nalezli jsme významné rozdíly (p<0.001) v podílu úmrtí, počtu vyžadovaných operací pankreatu a délce hospitalizace mezi všemi kategoriemi tíže dle revidované atlantské klasifikace. Podíl úmrtí v kategoriích tíže dle revidované atlantské klasifikace byl 0% u lehké AP, 15,9% u středně těžké AP a 57,2% u těžké AP. 12 z 19 pacientů souboru zemřelo následkem septických komplikací. Počet leukocytů a RDW byly identifikovány jako významný prediktor tíže AP a RDW také významný prediktor úmrtí u AP v souboru. Incidence AP v regionu na sledovaném území činila 10,04 případů/100tisíc obyvatel ročně. Nejzastoupeněnjší etiologii byla biliární AP s 43,3% případů. Podíl úmrtí ve věkové skupině do 40 let byl 2,7%, ve věkové skupině 40-60 let byl podíl úmrtí 10,8%, zatímco ve věkové skupině nad 60 let byl podíl úmrtí až 18,2 %. Závěr: Na souboru 159 pacientů z terciárního centra jsme validizovali kategorie tíže AP definované podle revidované atlantské klasifikace. Infekční komplikace se významně podílí v úmrtnosti na středně těžkou i těžkou AP. Počet leukocytů a RDW v první den hospitalizace se jeví jako potencionální prognostické markery tíže a rizika úmrtí u AP. Česká republika patří ke krajinám s mírnou predominancí biliární etiologie u AP. Nalezli jsme významnou korelaci podílu úmrtí s rostoucím věkem pacientů. Abstract:

Background: Acute pancreatitis (AP) is a serious condition accompanied with important mortality and morbidity and moreover with increasing incidence all around the world. Universal and clearly embedded classification of the AP provides a basis for a communication between clinicians as well as for comparative clinical studies. Understanding of the epidemiological chracteristics of the disease and discovery of the reliable severity markers will help in finding more effective proceedings in decision making and therapy. Methods: We analyzed 159 patients with AP hospitalized in tertiary centre. We categorized the patients as per both original Atlanta and revised Atlanta classification. According to analyzed outcomes we justified the severity categories. We evaluated BMI, leucocyte count and red cell distribution width (RDW) and their ability to predict severe form or death in AP. In a study group of 261 consecutive patients from multiple centres in South Moravian region we analyzed incidence, etiology, case fatality and therapy of AP. Results: We found significant differences (p<0.001) in death rates, number of required operations of pancreas and total hospital stay length between all severity categories defined by revised Atlanta classification. Death rate in the severity categories were 0% for mild, 15.9% for moderately severe and 57.1% for severe AP. 12 out of 19 patients in the study group died because of septic complications. Leucocyte count and RDW were identified as significant predictors for severe AP, and RDW was also identified as significant predictor for mortality in patients with AP in our study group. We obtained incidence of 10.04 /100000 inhabitants and year in the study region. The most frequent etiology was biliary with 43.3% of cases. The case fatality rate in the age group under 40 years old was 2.7% while 10.8% and 18.2% of patients died in the age groups of 40-60 years old, and older than 60 years respectively. Conclusions: We validated severity categories defined by revised Atlanta classification on a study group of 159 patients with AP from the tertiary centre. Infectious complications were identified as important causes of death in both moderately severe and severe categories. Leucocyte count and RDW on the first day of hospital stay needs to be confirmed as a potential prognostic marker of severity and risk of death in patients with AP. The Czech Republic belongs to the countries with a slight predominance of biliary AP. We found a significant correlation between increasing age and death in patients with AP.
