Advanced imaging methods in patients with non-lesional epilepsy

Abstract:

Dizertační práce shrnuje poznatky o využití pokročilých zobrazovacích metod ke studiu a diagnostice epilepsie. Důraz je kladen na tzv. MR-negativní pacienty, tj. paci-enty bez viditelných strukturních změn, kde může být přínos těchto metod nejvyšší. Po stručném vysvětlení pojmů a vhledu do projevů epilepsie je teoretická část roz-dělena do dvou částí, na strukturní zobrazování a funkční zobrazování. Strukturní zobrazování se věnuje především morfometrickým metodám a difuznímu zobrazo-vání pomocí magnetické rezonance (MR), funkční zobrazování potom rozebírá me-tody využívající MR (fMRI a ASL), PET a SPECT. Úvod je zakončen nástinem možnos-tí, jak na základě dílčích výsledků predikovat lokalizaci SOZ. Empirická část práce je založena na pěti dříve publikovaných recenzovaných publikacích. První publikace se zabývá adaptivním prahováním zobrazovacích výsledků při individuálním vyšetřo-vání pacientů. Druhá studie srovnává přínosnost zcela neinvazivního MR ASL a PET využívajícího ionizující záření. Třetí studie spojuje ověřené difuzní mikrostrukturní parametry do nového, více senzitivního a specifického parametru. Čtvrtá studie ob-sáhle srovnává přesnost vybraných metod ve skupině úspěšně operovaných MR-negativních pacientů. A pátá studie se zabývá tvorbou matematického modelu pro automatickou fúzi dílčích výsledků do jedné co nejpřesnější predikční mapy. V závě-ru je diskutováno, že pokročilé metody a post-processingové analýzy jsou u MR-negativních pacientů přínosné při odhalování jinak neviditelných abnormit ukazují-cích na SOZ a měly by být zahrnuty mezi metody zvažované při předoperačním hod-nocení.

Abstract:

This dissertation summarizes findings on the use of advanced imaging methods in the study and diagnosis of epilepsy. The focus is on so-called MR-negative patients, i.e. patients without visible structural changes, where the benefit of these methods may be greatest. The theoretical part is divided into a brief explanation of the con-cepts and an insight into the manifestations of epilepsy, structural imaging and func-tional imaging. Structural imaging focuses primarily on morphometric methods and diffusion imaging using magnetic resonance (MR), whereas functional imaging ana-lyzes methods using MR (fMRI and ASL), PET, and SPECT. The theoretical part ends with an overview of the possibilities of predicting the localization of the seizure on-set zone based on individual results. The empirical part of the work is based on five previously published peer-reviewed papers. The first paper deals with adaptive thresholding of imaging results during individual patient examinations. The second study compares the utility of completely non-invasive MR ASL and PET using ioniz-ing radiation. The third study combines proven diffuse microstructural parameters into a new, more sensitive and specific metric. The fourth study comprehensively compares the accuracy of selected methods in a group of successfully operated MR-negative patients. And the fifth study deals with the creation of a mathematical mod-el for the automatic fusion of individual methods into a SOZ prediction map as accu-rate as possible. In conclusion, it is discussed that advanced methods and post-processing analyses are beneficial in detecting otherwise invisible abnormalities in MR-negative patients and should be included among the methods considered in preoperative evaluation.
