Diagnosis of language abilities in children with developmental language disorder

Abstract:

Disertační práce se zabývá psychometrickým přístupem v diagnostice jazykových schopností u dětí s vývojovou dysfázií na přelomu předškolního a mladšího školního věku. Cílem předkládané disertační práce je diagnosti-kovat úroveň jazykových schopností u dětí s vývojovou dysfázií v porovnání s dětmi s typickým jazykovým vývojem. Teoretická část disertační práce je rozdělena do tří kapitol. První ka-pitola je úvodem do problematiky, kde jsou představeny aktuální přístupy a jsou diskutovány změny, které přináší nová klasifikace. Dále jsou popsány terminologické změny a nová ustanovení, která přináší mezinárodní konsor-cium. Je také definován charakter a podoba vývojové dysfázie. Představena je také etiologie vývojové dysfázie a konfrontována s výzkumnými zjištění-mi, zejména na poli genetiky. Na etiologii navazuje překrývání se vývojové dysfázie s dalšími diagnózami, které mají často podobné symptomy. Druhá kapitola se věnuje symptomům vývojové dysfázie v kontextu jazykového vývoje, zejména v předškolním a mladším školním věku. Jsou zde popisová-ny jazykové struktury, které bývají zasaženy vývojovou dysfázií a u nichž se nejčastěji objevují symptomy, společné pro tento typ postižení. Pozornost je také věnována dopadu vývojové dysfázie na edukaci jedince. Třetí kapitola se zabývá diagnostikou jazykových schopností. V úvodu je představen cha-rakter diagnostického procesu a průběh diagnostiky vývojové dysfázie v České republice. Dále jsou popsány testové baterie, které jsou ve světě standardizovány a využívány jako vhodné indikátory vývojové dysfázie. V této části je také nastíněná situace v České republice a na Slovensku. Zá-věr kapitoly je věnovaný důležitosti včasné rané identifikaci rizikových skupin, jako faktoru možného záchytu projevů vývojové dysfázie a efektivnosti ve využití zejména rodičovských dotazníků. Výzkumná část je představena ve dvou hlavních kapitolách. Jedná se o metodologii výzkumného šetření a výsledky výzkumného šetření. Čtvrtá kapitola představuje metodologii výzkumného šetření ve výzkumném projektu. Dále jsou představeny zvolené výzkumné cíle, parciální cíle a hypotézy. Na tuto část navazuje popis etapy výzkumné-ho šetření, výběr participantů a model průběhu výzkumného šetření s participanty. Následně jsou popsány standardizované i nestandardizované testové baterie, které byly ve výzkumném projektu využívány. Pátá kapitola představuje prezentaci výsledků výzkumného šetření v preferenčních oblas-tech našeho zájmu, tedy v testu receptivní slovní zásoby, testu zkoumající fungování krátkodobé fonologické paměti a v testu zkoumající fungování centrální exekutivy. Výzkumný soubor tvořilo 126 participantů. Experi-mentální skupinu tvořilo 63 dětí s vývojovou dysfázií a kontrolní skupinu tvořilo 63 dětí s typickým jazykovým vývojem. Děti v kontrolní skupině byly záměrně mladší v průměru o dva roky. V závěrech šetření jsou prezen-továny výsledky výzkumu zaměřené na úroveň jazykových schopností u dětí s vývojovou dysfázií a u dětí s typickým jazykovým vývojem. V rámci vzta-hových analýz byl aplikován dle preferencí rozdíl průměrů ve skupinách (dif.), dvouvýběrový T-test pro nezávislé výběry (Independent Samples T-test), Pearsonův korelační koeficient (rp), 95 % interval spolehlivosti (95 % Confidence Interval of the Difference; 95 % CI) a případně hladina statistické signifikance (p.) Jsou představeny výsledky dosažených skórů v jednotlivých testových úlohách, vliv věku na úspěšnost v testových úlo-hách a vzájemné vztahy mezi jednotlivými testovými úlohami u dětí s vývojovou dysfázií a u dětí s typickým

jazykovým vývojem. Posledními kapitolami jsou diskuse a závěr, kde jsou shrnuty dosažená zjištění a výsledky v kontextu vědeckých studií. Popsány jsou také možné limity výzkumu a přínos či doporučení pro teorii a praxi.

Abstract:

The dissertation deals with a psychometric approach in the diagnosis of language abilities in children with developmental language disorder at the turn of preschool and younger school age. The aim of the present disserta-tion is to diagnose the level of language abilities in children with develop-mental language disorder in comparison with children with typical language development. The theoretical part of the dissertation is divided into three chapters. The first chapter is an introduction to the topic, where we present current approaches and perspectives on the diagnosis of developmental language disorder. We discuss the changes brought about by the new classifications, the change in terminology and the related attention in the scientific field that the field has lacked. We also elaborate on the terminology changes and new provisions brought about by the international consortium. We describe the nature and form of developmental language disorder. We also present the etiology of developmental language disorder and confront research find-ings, particularly in the field of genetics. The etiology is followed by the overlap of developmental language disorder with other diagnoses, which often have similar symptoms. The second chapter focuses on the symptoms of developmental language disorder in the context of language develop-ment, particularly in preschool and younger school-age children. We de-scribe the language structures that tend to be affected by developmental dysphasia and which most often show symptoms common to this type of disability. We also address the impact of developmental language disorder on the education of the individual. In the third chapter, we discuss the diag-nosis of language abilities. In the introduction we present the nature of di-agnosis and the course of diagnosis of developmental language disorder in the Czech Republic. Then we describe the test batteries that are standard-ized in the world and used as suitable indicators of developmental dyspha-sia. We also describe and present the situation in the Czech Republic and Slovakia. At the end of the chapter, we discuss the importance of early di-agnosis in at-risk children as a factor for early detection of developmental language disorder and the effectiveness of the use of parental question-naires. We present the research part in three parts. These are the research objectives and hypotheses, the research methodology and the results of the research investigation. In chapter four, we present the research objectives and hypotheses. In chapter five, we describe the methodology of the research investigation in our pro-ject. We also describe the stages of the research inquiry, the selection of the participants and the model of the research inquiry process with the partici-pants. We then present the standardized and non-standardized test batteries that were used in the research project. The sixth chapter presents the presen-tation of the results of the research investigation in our preferential areas of interest, i.e., the receptive vocabulary test, the test investigating the func-tioning of short-term phonological memory, and the test investigating the functioning of the central executive. The research population consisted of 126 participants. The experimental group consisted of 63 children with de-velopmental language disorder and the control group consisted of 63 chil-dren with typical language development. The children in the control group were intentionally younger by an average of two years. The main and sub-objectives of the research were stated, and hypotheses were presented. In the conclusions of the investigation, we present the results of the research focused on the level of self-linguistic abilities in children with developmen-tal language disorder and in children with typical language development. The last chapters are the discussion and conclusion, where we summarize the findings and results in the context of scientific studies. We also describe possible research limities and contributions or recommendations for theory and practice.
