The role of motor function assessment in the diagnosis and treatment of neuromuscular and spondylogenic disorders

Abstract:

Hlavní témou této disertační práce je svalový systém trupu a jeho role ve zdraví páteře. Propojuje dvě různá neurologická onemocnění, neuromuskulární a verebrogenní a to skrze předpokládanou dysfunkci svalstva osového systému, kterou bylo třeba objektivizovat. Úvodní část práce podává přehled neurologických poruch, které byly cílem studií, nabízí širokou škálu adekvátních vyšetřovacích nástrojů, diskutuje jejich výhody a nevýhody a ukazuje jejich důsledky pro terapii. Hlavní zjištění studií zahrnutých do doktorské práce jsou: Opakovatelnost a krátkodobá i dlouhodobá spolehlivost technik měření maximální izometrické síly extenzorů dolní části zad pomocí ručního dynamometru ve třech posturálních polohách byla potvrzena jako vynikající. Byla definována baterie jednoduchých testů, které lze snadno provádět v každodenní praxi a které poskytují komplexní přehled o základních složkách funkce trupu (o síle a svalové vytrvalosti). Adherence k domácímů rehabilitačnímu programu u pacientů s chronickou nespecifickou bolestí dolní části zad byla při použití telemonitoringu vynikající; měření orientovaná na pacienta a měření motorických funkcí (výše zmíněnou baterii testů) ukázala, že pozitivní účinek intervence byl významný. Pacienti s myotonickou dystrofií typu 2 (MD2) vykazovali významnou dysfunkci svalů trupu, včetně svalů dýchacích, což se projevilo snížením svalové síly a vytrvalosti. Prevalence chronické bolesti dolní části zad (CLBP) u pacientů s MD2 přesahovala padesát procent a bylo zjištěno, že jediným významným rizikovým faktorem pro CLBP u těchto pacientů je snížená maximální izometrická síla extenzorů dolní části zad v poloze na břichu. Kromě diagnostického nástroje, který byl výsledkem studia, byly vyvinuty a do klinické praxe zavedeny dva rehabilitační programy.

Abstract:

This doctoral thesis focuses primarily on the trunk muscle system and its role in spinal health. It links two different neurological disorders, neuromuscular and spondylogenic, through the assumed axial muscle dysfunction that needed to be ob-jectified. The introductory part of the paper gives an overview of the neurological disorders that were the target of the studies, offers a wide range of adequate out-come measures, discusses their pros and cons, and shows their implications for therapy. The main findings of the commented studies are: The repeatability and short-term and long-term test-retest reliability of techniques for measuring maximal isometric lower back extensor strength using a hand-held dynamometer in three postural positions have been confirmed to be excellent. A battery of simple tests that are easy to perform in everyday practice and provide a comprehensive overview of essential components of trunk function (strength and muscular endurance) has been defined. Adherence to the home-based rehabilitation programme in patients with chronic non-specific low back pain was excellent using telemonitoring; the pa-tient-oriented measures and the measures of motor function (the test battery men-tioned above) showed that the positive effect of the intervention was significant. Pa-tients with myotonic dystrophy type 2 (MD2) showed significant dysfunction of the trunk muscles, including the respiratory muscles, as evidenced by decreased muscle strength and endurance. The prevalence of chronic low back pain (CLBP) in patients with MD2 exceeded fifty percent, and the only significant risk factor for CLBP in these patients was found to be decreased maximal isometric low back extensor strength in the prone position. In addition to the diagnostic tool (battery of motor tests) that resulted from the PhD study, two rehabilitation programmes have been developed and introduced into clinical practice.
