Physiotherapy in a patient after leukemia with a focus on gait stereotype

Abstract:

Úvod: Leukémie představuje nejčastější formu rakoviny u dětí a dospívajících. Ačkoli incidence roste, zlepšuje se prognóza a snižuje se mortalita. Vzhledem k tomuto zlepšení míry přežití se pozornost přesunula k pochopení nepříznivých dopadů onkologické léčby. Nežádoucí účinky léčby a s ní spojené komplikace mají mimo jiné vliv na pohybový aparát. Cílená fyzioterapie může příznivě ovlivnit vzniklé patologie v pohybových stereotypech včetně stereotypu chůze. Cíl práce: Cílem této práce je zpracovat teoretické poznatky související s diagnózou akutní lymfoblastické leukémie a přispět k řešení problematiky patologického stereotypu chůze u pacientů s tímto onemocněním. Metodika: Přehled teoretických poznatků je vypracován formou rešerše a analýzou odborné literatury. Hlavní část obsahuje kazuistiku pacientky po prodělané leukémii se zaměřením na správný stereotyp chůze. Zahrnuje anamnézu, vstupní kineziologicky rozbor, na jehož základě je sestaven krátkodobý fyzioterapeutický plán terapie. Dále je popsán průběh fyzioterapeutických intervencí, kdy jednotlivé terapie probíhaly 1krát týdně na 45 minut, po dobu osmi týdnů. Ve fyzioterapeutických intervencích se využívají fyzioterapeutické metody a koncepty, kterými jsou např. technika měkkých tkání, postfacilitační strečink, mobilizační techniky, aktivní cvičení proti odporu s využitím therabandu, senzomotorická stimulace, proprioceptivní neuromuskulární facilitace a akrální koaktivační terapie. Nedílnou součástí je také edukace a autoterapie. V závěru je proveden výstupní kineziologický rozbor a následně porovnání vstupních a výstupních dat z kineziologických rozborů s výsledným dlouhodobým fyzioterapeutickým plánem. Výsledky: Výsledky této práce poukazují na příznivé ovlivnění reflexních změn a celkové zlepšení pohybového stereotypu chůze pomocí cílené fyzioterapie u pacientky s diagnózou akutní lymfoblastické leukémie. Došlo k úpravě postavení decentrovaných kloubů dolních končetin a napřímení celého osového aparátu. Vestoje pacientka lépe zatěžuje styčné body na chodidle. Při chůzi u pacientky nastaly změny v postupném zatěžování a odvíjení plosky v jednotlivých segmentech nohy. Závěry: Práce pro mě byla prinosna a obohatila me o nove znalosti. Měla jsem možnost uplatnit teoreticke i prakticke dovednosti, ktere jsem ziskala během studia.

Abstract:

Introduction: Leukaemia is the most common form of cancer in children and adolescents. Although the incidence is increasing, prognosis is improving and mortality is decreasing. Given this improvement in survival rates, attention has shifted to understanding the adverse effects of cancer treatment. The side effects of treatment and associated complications affect, among other things, the musculoskeletal system. Specific physiotherapy can positively influence the resulting pathologies in movement stereotypes, including gait stereotypes. The objective: The aim of this thesis is to review the theoretical knowledge related to the diagnosis of acute lymphoblastic leukemia and to contribute to the solution of the problem of pathological gait stereotype in patients with this disease. Methodology: The review of theoretical information is developed by searching and analysing the literature. The main part contains a case report of a patient after leukaemia with a focus on the correct walking stereotype. It includes anamnesis, initial kinesiological assessment, on the basis of which a short-term physiotherapy plan is established. The physical therapy interventions are also described, with individual therapy sessions occurring 1 time per week for 45 minutes, for a period of eight weeks. Physiotherapy methods and concepts used in the physiotherapy interventions include soft tissue techniques, post-facilitation stretching, mobilisation techniques, active resistance exercise using theraband, sensorimotor stimulation, proprioceptive neuromuscular facilitation and acral coactivation therapy. Education and self-therapy are also an integral part. Finally, an outcome kinesiological assessment is performed, followed by a comparison of input and output kinesiological data with the resulting long-term physiotherapy plan. Results: The results of this study indicate a beneficial effect on reflex changes and overall improvement of gait stereotype by specific physiotherapy in a patient with a diagnosis of acute lymphoblastic leukaemia. There was a modification of the position of the decentered joints of the lower limbs and a straightening of the entire axial apparatus. In standing, the patient better loads the contact points of the foot. During gait, the patient experienced changes in the progressive loading and unloading of the plantarflexion in each segment of the foot. Conclusions: The work has been beneficial for me and has enriched me with new knowledge. I had the opportunity to apply the theoretical and practical skills I have gained during my studies.
