Effect of ambulatory physiotherapy on gait in patients with functional neurological movement disorder

Abstract:

Diplomová práce zkoumá vliv ambulantní fyzioterapie na pacienty s funkčními poruchami chůze, se zaměřením na objektivní a subjektivní hodnocení výsledků léčby. Objektivní hodnocení funkční disability bylo provedeno s využitím škály SFMDRS, které prokázalo statisticky významné zlepšení. Subjektivní vnímání kvality života bylo hodnoceno pomocí dotazníku SF-36, kde nebyly zjištěny statisticky významné změny ve zdravotním stavu, což naznačuje možnost, že pacienti si své zlepšení subjektivně neuvědomují. Výzkumný soubor tvořilo celkem 13 pacientů s průměrným věkem 44 let, kteří absolvovali 5 až 17 sezení fyzioterapie. Studie neprokázala statistickou významnost mezi délkou trvání fyzioterapie nebo počtem terapií a zlepšením ve zdravotním stavu, ačkoli byl pozorován mírně pozitivní trend u počtu terapií. Také mezi délkou trvání příznaků a změnou ve skóre SFMDRS nebyla nalezena statistická významnost, avšak byla zaznamenána slabá tendence ke zlepšení subjektivního vnímání s mírným zlepšením objektivních hodnot. Tato práce potvrzuje význam ambulantní fyzioterapie v léčbě funkčních poruch chůze.

Abstract:

The diploma thesis examines the impact of outpatient physiotherapy on patients with functional gait disorders, focusing on both objective and subjective evaluation of treatment outcomes. Objective evaluation of functional disability was conducted using the SFMDRS scale, which revealed statistically significant improvements. Sub-jective perception of quality of life was assessed using the SF-36 questionnaire, where no statistically significant changes in health status were found, suggesting that patients might overestimate their improvement. The research sample consisted of 13 patients with an average age of 44 years, who underwent 5 to 17 physiotherapy sessions. The study did not demonstrate sta-tistical significance between the duration of physiotherapy or the number of therapy sessions and improvements in health status, although a slightly positive trend was observed in the number of therapies. Likewise, no statistical significance was found between the duration of symptoms and changes in the SFMDRS score, but a weak tendency towards improvement in subjective perception with slight improvements in objective values was recorded. This work confirms the importance of outpatient physiotherapy in the treatment of functional gait disorders.
