Colorectal cancer: Biology, therapy and biomarkers

Abstract:

Kolorektální karcinom je nádorové onemocnění tlustého střeva a konečníku s celosvětově vysokou incidencí a mortalitou. Tato bakalářská práce se v teoretické části věnuje léčbě metastatického kolorektálního karcinomu a roli relevantních faktorů jako mutace RAS, BRAF a mikrosatelitové nestability. Cílem experimentální části práce bylo identifikovat biologicky relevantní biomarkery odpovědi metastatického kolorektálního karcinomu (mCRC) na léčbu tyrosinkinasovým inhibitorem bevacizumabem s využitím proteomové analýzy 33 nádorových tkání, z nichž část odpovídala na léčbu bevacizumabem a část nikoliv, a 11 přilehlých nenádorových tkání. Pomocí vytvořené spektrální knihovny bylo na bázi kapalinové chromatografie a hmotnostní spektrometrie s datově nezávislým sběrem dat (LC-DIA-MS/MS) kvantifikováno 9299 proteinových skupin, z nichž se pomocí statistických modelů podařilo identifikovat jeden potenciální diagnostický marker (ING1), jenž by mohl sloužit k predikci úspěšnosti léčby bevacizumabem, a dva potenciální alternativní terapeutické cíle (NOTCH2 a TROP2). U NOTCH2 a TROP2 byla provedena korelační analýza, ze které vyplynulo, že oba proteiny vzájemně nekorelují. Hladina TROP2 byla s využitím westernblotingu s imunochemickou detekcí porovnána v rámci panelu 7 CRC linií a srovnána s MS daty z veřejně dostupné databáze, přičemž byla zjištěna hraniční korelace mezi oběma přístupy. Výsledky dále naznačují, že hladina TROP2 nesouvisí se statusem KRAS genu. Data poskytují robustní základ pro validaci závěrů této pilotní studie na větších souborech pacientů.

Abstract:

Colorectal cancer involves cancer of the colon and rectum with a high incidence and mortality worldwide. The theoretical part of this thesis focuses on the treatment of metastatic colorectal cancer and the role of relevant factors such as RAS mutations, BRAF and microsatellite instability. The aim of the experimental part of the thesis was to identify biologically relevant biomarkers of response of metastatic colorectal cancer (mCRC) to treatment with the tyrosine inhibitor bevacizumab using proteomic analysis of 33 tumor tissues, some of which responded to bevacizumab treatment and some of which did not, and 11 adjacent non-tumor tissues. Using the established spectral library, 9299 protein groups were quantified by liquid chromatography-mass spectrometry with data independent data acquisition (LC-DIA-MS/MS), from which one potential diagnostic marker (ING1) that could be used to predict the success of bevacizumab treatment and two potential alternative therapeutic targets (NOTCH2 and TROP2) were identified using statistical models. Correlation analysis was performed for NOTCH2 and TROP2 and showed that the two proteins do not correlate with each other. Using western blotting with immunochemical detection, TROP2 levels were compared within a panel of 7 CRC lines and compared with MS data from a publicly available database, resulting in a borderline correlation between these two approaches. The results suggest that TROP2 levels are not related to KRAS gene status. The data provide a robust basis for validating the conclusions of this pilot study in larger patient cohorts.
