Experiences of nicotine sachets and other forms of tobacco use among primary and secondary school students

Abstract:

Úvod: Na českém trhu se od roku 2019 objevují nové nehořlavé výrobky obsahující nikotin - nikotinové sáčky. I když se nikotin tímto způsobem vstřebává pomaleji než při kouření klasických cigaret, jsou zde zdravotní rizika, např. předávkování a negativní vliv na ústní dutinu. Cílem této práce bylo zjistit rozsah užívání nikotinových sáčků a dalších nikotinových produktů mezi studenty základních a středních škol v Moravskoslezském kraji, a provedení analýzy dat edukačního programu "Prevence kouření". Cílem byla analýza faktorů, které ovlivňují znalosti o rizicích, která jsou spojena s kouřením. Metodika: Dotazníkové šetření probíhalo v rámci edukační akce "Prevence kouření" pod záštitou ZÚ Ostrava a LF OU v období 2022-2024. Dále byla vyhodnocena efektivita preventivního programu "Prevence kouření" za období 2021-2023. Pro popis získaných výsledků byla použita popisná statistika, statistické testy (chí-kvadrát a Kruskal-Wallisův test, Spearmanův korelační koeficient) a model logistické regrese. Srovnání efektivity preventivního programu bylo provedeno neparametrickým Wilcoxonovým párovým testem na hladině významnosti 5 %. Pro zpracování dat byl použit program Stata verze 16. Výsledky: Celkem bylo zapojeno 23 škol a 959 žáků v průměrném věku 13,3?1,9 let. Bylo zjištěno, že 21,4 % žáků mělo zkušenost s užíváním nikotinových sáčků - 30,5 % chlapců a 12,5 % dívek (p <0,001). Celkem 31 % dětí získalo zkušenost před dovršením 12 let. Pravidelně (1krát a častěji za týden) nikotinové sáčky užívá 5 % žáků. Hlavní zdroje informací o sáčcích jsou internet a kamarádi. Z dalších výrobků žáci užívají pravidelně elektronické (15 %) a klasické (8 %) cigarety, vodní dýmky (4 %) a nahřívaný tabák (2 %). Efektivita vzdělávací akce byla hodnocena pomocí dotazníku před a po edukaci. V rámci edukační akce došlo po edukaci ke zvýšení průměrného počtu bodů z 10,5 na 14,9. Po edukaci došlo ke statisticky významnému zlepšení průměrných znalostí z 58,3 % na 82,8 % (p <0,001). Závěr: Více než pětina žáků ve věku kolem 13 let má nějakou zkušenost s užíváním nikotinových sáčků. Významně vyšší riziko užívaní je u chlapců a u kuřáků klasických nebo elektronických cigaret. Edukací na téma kouření došlo k navýšení znalostí o 24,5 %, můžeme hodnotit akci jako efektivní.

Abstract:

Introduction: New non-flammable products containing nicotine - nicotine pouches (NP) - appeared on the Czech market in 2019. Although nicotine is absorbed more slowly than in cigarettes, there are health risks such as overdose and negative effects on the oral cavity. The aim of this study was to determine the extent of the use of NP and other nicotine products among primary and secondary school students in the Moravian-Silesian region and to perform data analysis of the Smoking Prevention Education Programme. The aim was to analyse factors that influence knowledge about risks associated with smoking. Methodology: The survey was conducted within the framework of the educational event "Smoking Prevention" under the auspices of the Ostrava ZÚ and LF OU in the period of 2022-2024. Furthermore, the effectiveness of the preventive programme "Smoking Prevention" for the period 2021-2023 was evaluated. Descriptive statistics, statistical tests (chi-square and Kruskal-Wallis test, Spearman's correlation coefficient) and logistic regression model (alpha = 5 %) were used to analysethe obtained results. Comparison of the effectiveness of the prevention programme was performed by non-parametric Wilcoxon paired test at the significance level of 5 %. Stata version 16 was used for data processing. Results: A total of 23 schools and 959 pupils with an average age of 13.3?1.9 years were involved. It was found that 21.4 % of pupils had experience of nicotine pouches use - 30.5 % of boys and 12.5 % of girls (p <0.001). A total of 31 % of children had gained experience by the age of 12 years. 5 % of pupils used nicotine pouches regularly (i.e. 1 or more times per week). Internet and friends are the main sources of information about pouches. Among other products, pupils regularly use electronic (15 %) and conventional (8 %) cigarettes, hookahs (4 %) and heated tobacco (2 %). The effectiveness of the educational event was evaluated using a questionnaire before and after the education. There was an increase in the mean score from 10.5 to 14.9 after the educational event. After education, there was a statistically significant improvement in mean knowledge from 58.3 % to 82.8 % (p <0.001). Conclusion: More than a fifth of pupils aged around 13 have some experience of using nicotine sachets. The risk of use is significantly higher among boys and among smokers of conventional or electronic cigarettes. The education on smoking increased knowledge by 24.5 %, therefore, we can evaluate the event as effective
