Signficance of microRNAs in molecular pathology of colorectal cancer

Abstract:

Východisko: Kolorektální karcinom se řadí mezi nejrozšířenější typy nádorů s pomalu klesající incidencí a stále narůstající prevalencí. Karcinom kolorekta charakterizuje koncept modelu vícestupňového vývoje zhoubného nádoru, ve kterém jsou nejdříve nahromaděny genetické změny zapříčiněné exogenními a endogenními faktory a posléze vzniká tumor. Zvýšená nebo snížená úroveň exprese microRNA (nekódující úseky RNA posttranskripčně regulující expresi genů) je důležitým faktorem vzniku kolorektálního karcinomu. Metody: V této studii se stanovovala hladina miR-21 v 56 vzorcích kolorektálního karcinomu a na stejném počtu tkání nenádorových od identických pacientů. Stanovení bylo provedeno metodou kvantitativní zpětné PCR v reálném čase. Exprese miR-21 byla poté srovnána v nádorové a nenádorové tkáni. Dále byly srovnány hladiny miR-21 v různých klinických stádiích a gradech CRC a nakonec bylo vyhodnoceno přežívání pacientů vzhledem k expresi miR-21. Výsledky: Mezi hladinou miR-21 ve zdravé tkáni a tkáni zasažené tumorem byl nalezen signifikantní rozdíl. Hladina miR-21 (P=0,0001) byla zvýšená (přibližně čtyřikrát) a tento údaj je statisticky významný. Dalším výsledkem bylo, že s narůstajícím klinickým stádiem CRC exprese miR-21 vykazuje vzrůstající trend, který však není statisticky významný. Grading u CRC nemá žádný vztah s hladinou miR-21 v nádorové tkáni. A nakonec z výsledků vlivu miR-21 na celkové přežívání pacientů CRC můžeme vyvodit závěr, že miR-21 je významným prognostickým markerem nádorů tlustého střeva a konečníku. Závěr: Tento výzkum potvrzuje výraznou změnu v expresi miR-21 u kolorektálního karcinomu. Tento poznatek udává jejich nezanedbatelný význam v molekulární patologii nádoru tlustého střeva a konečníku. Další pozorování ukázalo, že na hladině miR-21 závisí délka přežívání pacientů s tímto onemocněním. Studie potvrzuje výsledky dřívějších výzkumů, ve kterých se miR-21 ukazuje jako důležitý prognostický marker.

Abstract:

Objectives: Colorectal carcinoma ranks among the most wide-spread types of tumours with a slowly decreasing incidence and constantly growing prevalence. Colorectal carcinoma is characterised by the concept of the model of the malign tumour multi-stage development in which genetic changes caused by exogenous and endogenous factors are accumulated with the eventual tumour growth. The increased or decreased level of the expression of micro RNA (non-coding RNA segments with the post-transcription gene expression control) is a significant factor of colorectal carcinoma nascence. Methods: In this study the miR-21 level was determined in 56 colorectal carcinoma samples and in the same number of non-tumoral tissues of the identical patients. The determination was implemented by the method of quantitative regressive PCR in real time. The miR-21 expression was then compared in tumoral and non-tumoral tissues. Further, the miR-21 levels in various clinical stages and CRC grades were compared, and in the end the patients’ survival was evaluated in reference to miR-21expression. Results: A significant difference was found between the miR-21 levels in healthy and tumoral tissues. The level of miR-21 (P=0, 0001) was increased (approximately four times) and this data is statistically significant. Another result was that with the increasing clinical stage the CRC expression of miR-21 shows increasing trend which however is not statistically significant. CRC grading has no relation to miR-21 level in the tumoral tissue. Finally, from the results of miR-21 influence on general survival of the CRC patients we can deduce that miR-21 is a significant prognostic marker of colorectal carcinoma. Conclusion: This research confirms a distinct change in the miR-21 expression with colorectal carcinoma. This observation indicates their indispensable significance in molecular pathology of CRC tumours. Further study showed that the length of the CRC patients’ survival depends on the miR-21 level. The study confirms the results of the earlier research in which miR-21appears as an important prognostic marker.
