Cefalee di tipo tensivo: comparazione di due protocolli differenti di trattamento: massoterapia a confronto con massoterapia e trigger points

Tension-type headaches: comparison of two different treatment protocols: massotherapy compared to massotherapy and trigger points.

Abstract:

The research project I drafted was created with the intent to make a comparison with the results obtained, in terms of improvement in pain intensity between the two groups of patients with recurrent tension headache with similar characteristics among different patients. The sample was chosen to represent the population in order to carry out on them this study of scientific thesis. They were chosen patients consistent with the requirements necessary for the purposes of the study and that they represented the general population. These patients were randomized into two groups at random and subjected to two different rehabilitation protocols. The group (A) or control, to which they were assigned 10 subjects, was subjected to a cycle of 8 sessions of massage therapy of the spine as a whole, treating more muscles of the cervical spine and shoulder girdle, as it emerges from literature that these are the muscles that may contribute to headaches if contracts. Group (B) experimental have been assigned 10 patients treated also with 8 massage sessions. Were treated as in the first group the muscles of the cervical spine and shoulder girdle, but with the peculiarity to investigate and treat trigger points of the muscles of mastication to assess whether contractures of the latter may affect or cause the same symptoms in their patients. In order to use the information more 'reliable in the field were consulted the following databases (Medline, PubMed, pedro), with the addition of consulting books updated to the most' recent articles such as (D.Neumann, Okeson managment of temporomandibolars disorders and occlusion). Patients in both groups were asked about their quality and intensity of pain using the VAS (visual analogic scale); This scale was used to assess the effectiveness of treatment using the statistical survey T-student. The entire study was conducted over 4 weeks with a frequency of treatment bi-weekly, and have run a follow up to a month. The VAS scale was detected before and after the first treatment session, weekly and follow up to a month. Both groups examined have had positive response to the treatment, especially in group (B) experimental, treated with massage therapy and trigger points is shown a positive response to treatment immediate and more 'durable in time confirmed by the follow-up to a month. Contrary to the group (A) Control, treated only with massage therapy has employed more 'time to highlight improvements, and at follow up showed a slight recurrence of symptoms compared to the results obtained in the last session.

Il progetto di ricerca da me elaborato nasce con l?intento di effettuare un confronto con i risultati ottenuti, in termini di miglioramento dell?intensitŕ del dolore, tra due gruppi di soggetti affetti da cefalea tensiva ricorrente con caratteristiche simili tra i diversi pazienti. Il campione č stato scelto per rappresentare la popolazione al fine di poter eseguire su di essi questo studio di tesi scientifica. Sono stati scelti i pazienti congruenti ai requisiti necessari ai fini dello studio e che rappresentassero la popolazione generale. Tali pazienti sono stati randomizzati in maniera casuale in due gruppi e sottoposti a due protocolli riabilitativi differenti. Il gruppo (A) o di controllo, a cui sono stati assegnati 10 soggetti, č stato sottoposto ad un ciclo di 8 sedute di massoterapia del rachide nella sua globalitŕ, trattando maggiormente i muscoli del rachide cervicale e del cingolo scapolare, in quanto emerge dalla letteratura che questi sono i muscoli che possono contribuire a causare cefalee se contratti. Al gruppo (B) sperimentale sono stati assegnati 10 pazienti, trattati anch?essi con 8 sedute di massoterapia. Sono stati trattati come nel primo gruppo i muscoli del rachide cervicale e del cingolo scapolare, ma con la peculiaritŕ di indagare e trattare trigger points dei muscoli masticatori per valutare se contratture di quest?ultimi possono influire o provocare loro stessi sintomi ai pazienti. Al fine di utilizzare le informazioni piů attendibili in materia sono stati consultati i seguenti database (medline, pubmed, pedro), con l?aggiunta della consultazione di libri aggiornati ai piů recenti articoli quali (D.Neumann, Okeson managment of temporomandibolars disorders and occlusion). I pazienti di entrambi i gruppi sono stati interrogati sulla propria qualitŕ e intensitŕ del dolore utilizzando la scala VAS (Visual Analogic Scale); tale scala č stata utilizzata per valutare l?efficacia del trattamento utilizzando l?indagine statistica T-student. L?intero studio si č svolto nell?arco di 4 settimane con una frequenza di trattamento bi-settimanale, ed č stato eseguito un follow up ad un mese. La scala VAS č stata rilevata prima e dopo la prima seduta di trattamento, a cadenza settimanale ed al follow up ad un mese. Entrambi i gruppi presi in esame hanno avuto riscontro positivo al trattamento, in particolar modo nel gruppo (B) sperimentale, trattati con massoterapia e trigger points si č evidenziato una risposta positiva al trattamento immediata e piů duratura nel tempo confermata dal follow up ad un mese. Contrariamente, il gruppo (A) di controllo, trattato solo con massoterapia, ha impiegato piů tempo per evidenziare miglioramenti, ed al follow up ha registrato una leggera recidiva dei sintomi rispetto ai risultati ottenuti in ultima seduta.
