Physiotherapy for athletes with the swimmer´s shoulder syndrome

Abstract:

ABSTRAKT Cíl práce: Přispět k řešení problematiky fyzioterapie u sportovkyně se syndromem plaveckého ramene. Úvod: Práce se zabývá problematikou bolestivosti ramenního kloubu u závodních plavců. Tento stav je způsoben mnoha aspekty. Pohyb ve vodě klade velké nároky na maximální využití svalové síly, a to ve specifických pohybech a rozsazích. Tyto pohyby jsou pro ramenní kloub nezvyklé, a proto musíme dbát na důkladnou přípravu celého pohybového aparátu na tuto činnost. V opačném případě bude aparát neekonomicky zatížen a bude docházet k jeho postupné traumatizaci, vedoucí až k vyřazení sportovce z tréninkového procesu. Metodika: Tato práce je kazuistikou dvaceti dvouleté ženy, závodní plavkyně na elitní úrovni, která trpí potréninkovým bolestivým stavem ramenního kloubu. Bylo provedeno vstupní kineziologické vyšetření, na jehož základě jsem stanovila krátkodobý fyzioterapeutický plán. Cílem bylo snížení akutního bolestivého stavu probandky, uvolnění hypertonických svalů, odstranění trigger pointů, protažení zkrácených svalů, zvýšení svalové síly dolních fixátorů lopatek a uvolnění krční páteře. Během intervencí jsem používala fyzioterapeutické metody, jako jsou techniky měkkých tkání, myofasicální techniky, tejpování, PIR, centrace ramenního kloubu dle J. Čápové, koncept Spiraldynamik ?, PNF. Spolupráce probíhala po dobu dvou měsíců v počtu celkem deseti návštěv. Terapie probíhaly v prostředí bazénu Klíše - Ústí nad Labem. Během intervencí byla prováděna průběžná kontrolní vyšetření zaměřená na práci HSS (vždy po 2 týdnech). V polovině intervencí bylo provedeno kontrolní kineziologické vyšetření zaměřené na svalovou sílu a svalová zkrácení vybraných skupin. Výsledky: Z výsledků výstupního kineziologického rozboru vyplývá, že došlo k ústupu bolestivých potréninkových stavů ramenního kloubu, zvětšení svalové síly dolních fixátorů lopatek a zlepšení funkce HSS. Závěry: Došlo k subjektivnímu i objektivnímu zlepšení stavu probandky.

Abstract:

SUMMARY Objective: Contribute to the solution of an athlete's swimmer's shoulder syndrome Introduction: This work focuses on the matter of shoulder joint pain of competitive swimmers. Swimmer's shoulder is a term describing a painful state of the shoulder joint of these athletes. This state is caused by many aspects. Motion in water - swimming - demands maximum use of the power of the arm in specific movements and ranges. These movements are unusual for the shoulder joint. Therefore, we must pay attention to the whole motoric apparatus to be thoroughly prepared for this activity. In opposite case the apparatus would be stressed inefficiently and gradually traumatized, leading to discarding the athlete from the training process. Methodology: This work is a case study of a 22yo female competitive swimmer competing on an elite level, who is suffering with post-training painful state of her shoulder joint. An entrance case study has been made, based on which I have determined a short-term physiotherapeutic plan. The objective was to lower the acute painful state of the subject, relax the hypertonic muscles, remove the trigger points, stretch the shortened muscles and increase the lower scapulae fixator muscles and relax the cervical spine. During the interventions I have been using physiotherapeutic techniques such as soft tissue techniques, myofascial techniques, cupping, taping, PIR, shoulder joint centring by J. Čápová, Spiraldynamik? concept, PNF, Spiral stabilization. The interaction was taking place for two months in total amount of ten meetings. The interventions took place in the environment of Klíše swimming pool - Ústí nad Labem, mostly after finishing the afternoon training unit of the subject. In the course of the interventions, control examinations focused on the work of HSS were made (after each 2 weeks). In the middle of these interventions, a control kinesiological examination focused on the muscle strength and the muscle shortening of given muscle groups was made. Results: The output case study analysis indicates that the post-training painful states have relieved and the muscle strength of the lower scapulae fixator muscles has increased and function of HSS has improved. Conclusions: A subjective and objective improvement of the subject's state occurred.
