The distribution of depression of men and women with diagnosis a dependence on alcohol

Abstract:

Práce se zabývá tématem deprese mezi muži a ženami s diagnózou závislosti na alkoholu v kontextu přetrvávajícíh genderových stereotypů, podle kterých je deprese vnímána jako typická „ženská nemoc“. Statistiky mentálního zdraví, početné populační výzkumy i studie provedené na klinických vzorcích dokládají vyšší míru deprese u žen oproti mužům. Výzkumy byla dále prokázána vyšší míra deprese u osob závislých na alkoholu, než v populaci osob, které alkoholovou závislostí netrpí. Byl také prokázán silnější vztah mezi depresí a alkoholismem mezi ženami závislými na alkoholu oproti mužům závislým na alkoholu. Avšak na základě některých protikladných zjištění, jako je například vyšší míra sebevražd u mužů, je možné si položit otázku, zda zdravotní statistiky nepodhodnocují skutečný výskyt deprese u mužů oproti ženám. V teoretické části práce byly vymezeny psychiatrické poruchy, které jsou diagnostikovány jako deprese, závislost na alkoholu a duální diagnóza alkoholismu a deprese. Dále byl popsán jejich výskyt na základě údajů ze zdravotních statistik a výzkumů. Poté byla přiblížena teoretická vysvětlení rozdílů mezi muži a ženami v depresivitě, založená na sociálních a biologických faktorech. Výzkumná část práce vycházela z těch vysvětlení, která považují vyšší míru depresivity u žen oproti mužům za „ artefakt měření“. Tento fakt se pokoušejí vysvětlit tvrzením, že ženy vyjadřují své depresivní symptomy častěji než muži. Tato vyšší expresivita žen může být spojena s genderovými stereotypy. Výzkum byl založen na kvantitativní metodologii a byl rozdělen do dvou částí. V první výzkumné fázi byla testována hypotéza č. 1: Mezi muži a ženami s diagnózou závislosti na alkoholu z výzkumného vzorku č. 1 není signifikantní rozdíl v míře depresivity. V první části výzkumu byla předkládána Zungova sebeposuzovací stupnice deprese výzkumnému vzorku 108 pacientů, kteří se léčili na lůžkovém oddělení protialkoholní léčebny od října 2006 do září 2007 ( výzkumný vzorek č. 1). Ve druhé části výzkumu byly testovány dvě hypotézy. Hypotéza č. 2: Míra expresivity depresivních symptomů u osob s diagnózou závislosti na alkoholu z výzkumného vzorku č. 2 vůči osobám z jejich sociální sítě je vyšší u žen než u mužů. Hypotéza č. 3: Míra ochoty vyhledat odbornou pomoc (u lékaře, psychiatra, psychologa) při depresivních symptomech (které jsou měřeny Zungovou sebeposuzovací škálou deprese) u osob s diagnózou závislosti na alkoholu z výzkumného vzorku č. 2 je vyšší u žen než u mužů. Ve druhé části výzkumu byl předložen dotazník č. 2 o 20 položkách výzkumnému vzorku č. 2 . Tvořilo jej 52 pacientů s diagnózou závislosti na alkoholu, léčených na protialkoholním oddělení psychiatrické léčebny v době od září 2007 do listopadu 2007. Hypotéza č. 1 nebyla vyvrácena, hypotézy č. 2 a 3 nebyly potvrzeny. Muži a ženy s diagnózou závislosti na alkoholu (výzkumný vzorek č. 1) nevykazují signifikantní rozdíl v míře depresivních symptomů. Nebyl prokázán ani signifikantní rozdíl mezi muži a ženami z výzkumného vzorku č. 2 v míře deklarované ochoty svěřit se se svými depresivními symptomy osobám ze sociální sítě a vyhledat při nich odbornou pomoc. Tato zjištění však nemohou být vzhledem ke specifickému charakteru obou výzkumných vzorků zevšeobecněna na celou populaci. Muži a ženy s diagnózou závislosti na alkoholu v obou výzkumných vzorcích si mohli být pravděpodobně více podobní, než ženy a muži, kteří závislostí na alkoholu netrpí. Abstract:

This study deals with the issue of depression among men and women with the diagnosis of alcoholism in the context of predominant gender stereotype perceiving depression as “a typical female disorder”. Mental health statistics and numerous studies on both community and clinical samples have documented that women suffer from depression more than men. Studies have shown that the rate of depression in alcoholic population is higher than that of nonproblem-drinking population. Women alcoholics prove to have a stonger interrelation between alcohol and depression than men alcoholics. However, contradictory results such as higher rates of suicide among men raise the question of whether mental health statistics underestimate the true occurrence of depression among men and women. The theoretical part of the study focuses on psychiatric disorders diagnosed as depression, alcoholism, and dual diagnosis of depression and alcoholism. Also their occurrence is described. This is done on the base of mental health statistics and studies. Further, theoretical explanations of differences between depressive behavior of men and women are analysed on the background of social and biological factors. The research follows only those studies claiming that the higher rate of depression in women population is “a measurement artifact”. These studies explain this fact by claiming that women express the depressive symptoms more frequently than men. The expressivity might be related to gender stereotype. The research is based on quantitative investigation and is divided into two parts. In the first part is tested hypothesis No.1: There is no significant difference in the depression rate between men and women with diagnosis of alcoholism from the Sample No1. In the first part “Zung self-reported depression scale” is applied to sample of 108 inpatients with diagnosis of alcoholism. These patients went through psychiatric treatment from October 2006 to September 2007. In the second part of the research two hypotheses are tested. Hypothesis No.2 claims that: Women have stronger willingness to communicate symptoms of depression to members of their social network. Hypothesis No.3: Women have stronger willingness to look for professional help (by psychiatrist, psychologist, other medical doctor) when they exhibit symptoms of depression (measured by Zung self-reported depression scale). In the second part we work out a 20-item inventory questionnaire No.2 to Sample 2 of 52 inpatients with diagnosis of alcoholism, who were undergoing the psychiatric treatment from September 2007 to November 2007. Hypothesis 1 is confirmed, hypothesis 2 and 3 are not confirmed. Men and women with diagnosis of alcoholism (Sample No 1) do not show significantly different intensity of depression symptoms. Even a significant difference in the willingness to communicate their symptoms to their social environment and find professional help is not proved in the Sample 2. However, these conclusions cannot be generally extrapolated to the whole population due to a very specific nature of the sample. The men and women in the samples are probably more similar than men and women in a nonproblematic sample. The two Samples were not made randomly. But this study provides support for reconsideration of the assumption that depression is a typical women disorder. This research provides for some topics open for further investigation of gender differences in psychiatric morbidity.
