Gonarthrosis: comparison between manual therapy and physical therapy as a conservative instrument.

Abstract:

Knee osteoarthrosis is a chronic joint disease causing pain and disability. Physiotherapy is one of the most important components of therapy for osteoarthrosis of the knee. This study aimed to compare the efficacy of two different rehabilitation approaches in gonarthrosis: the physical therapy as iontophoresis and manual therapy(techniques of mobilization patellofemoral and tibiofemoral, traction techniques and exercises for muscle strengthening) compared for the treatment of painful symptoms in patients suffering from gonarthrosis. Purpose of the study is to demonstrate how manual therapy is more effective in knee OA symptoms. The selected patients were aged between 65 and 85 years, men and women, with no recent trauma, no concomitant pathology and lack of indication for surgical intervention. The clinical trial is made up of 20 participants randomly divided into the following two groups: Group 1 (n=10) received a iontophoresis therapy, Group 2 (n=10) received a physical manual therapy. Both groups received the supervised program for 3 months. The overall frequency choice was 3 times a month for patients treated with manual therapy(every 10 days), Each session has lasted 45 minutes with regard to the second group while the first group was effected a treatment every 7 days for a total of 4/5 treament in one month(20minuts each session).The drug chosen for the study as a medicine of iontophoresis is the Artrosilene. All the patients were evaluated using visual analogue scale (VAS) both, before and after the study. The results of the study showed a decrease of the VAS parameters in both groups, however, the findings indicated that manual techniques improved pain significantly more than physical therapy: following treatment, the Group 2 reported a significant decrease in the present pain severity compared to the Group 1 . 8 Tesi di laurea di Marco Giovagnoli Gonartrosi : terapia manuale e terapia fisica a confronto come strumento conservativo In conclusion the manual therapy protocol reduced pain significantly more than physical therapy protocol(group1 iontophoresis). A further study, with a greater number of patients, may be carried out to confirm what was found.

Abstract:

La gonartrosi č una malattia articolare cronica che provoca dolore e disabilitŕ. La Fisioterapia č parte fondamentale del trattamento della gonartrosi. Questo studio si propone di confrontare l'efficacia di due diversi approcci riabilitativi: l?utilizzo di terapia fisica come la ionoforesi e l'attuazione di tecniche manuali come mobilizzazione femoro-rotulea e femoro-tibiale, tecniche di trazione ed esercizi per il rafforzamento muscolare, messi a confronto, per il trattamento della sintomatologia dolorosa in pazienti affetti da gonartrosi. Scopo dello studio č quello di dimostrare come la terapia manuale č piů efficace rispetto alla terapia fisica(ionoforesi). I pazienti selezionati sono uomini e donne di etŕ compresa tra 65 e 85 anni,senza traumi recenti, assenza di malattie concomitanti e mancanza di indicazione chirurgica. Lo studio č composto da 20 partecipanti divisi casualmente in due gruppi : Gruppo 1 (n = 10) ha ricevuto terapia fisica( ionoforesi), Gruppo 2 (n = 10) ha ricevuto trattamento manuale con tecniche precedentemente menzionate. La durata dello studio č stata di 3 mesi. La frequenza con cui č stato attuato lo studio č stato di una volta alla settimana per una durata di 20 minuti a seduta per il gruppo 1. Il farmaco scelto č stato l?Artrosilene. Per quanto riguarda il gruppo 2 la frequenza č stata di una volta ogni 10 giorni per una durata di 45 minuti a seduta. Tutti i pazienti sono stati valutati mediante scala analogica visiva (VAS) sia all?inizio dello studio ,sia una volta terminato. I risultati dello studio hanno mostrato una diminuzione dei parametri VAS in entrambi i gruppi, tuttavia, i risultati indicano che le tecniche manuali sono da preferire rispetto alla terapia fisica: dopo il trattamento, il gruppo 2 ha riportato una diminuzione significativa del dolore sperimentato dai pazienti . In conclusione, i valori dei pazienti del gruppo 2 risultano significativamente ridotti rispetto a quelli del gruppo 1. Un ulteriore studio con un maggior numero di pazienti puň essere effettuato per confermare quanto trovato.
