Issues of care for invasive inputs in pediatric patients in palliative care

Abstract:

Tato práce se zaměřuje na péči o invazivní žilní vstupy u dětského pacienta v paliativní péči. V teoretické části je popsán vývoj paliativní péče ve světě a v České republice, zejména dětská paliativní péče. Dále jsou zde popsány žilní vstupy, které se nejčastěji používají u dětí vyžadujících paliativní péči a rizika spojená s jejich používáním. V empirické části jsou uvedeny výsledky literární rešerše a brainstormingu. Cílem výzkumu bylo pomocí metody FMEA (Failure Mode and Effects Analysis) identifikovat nejčastější rizika, která mohou nastat při ošetřování invazivního žilního vstupu u dětského pacienta při poskytování paliativní péče. Práce se zaměřila na rizika tunelizovaných centrálních žilních katétrů a portu. Hodnocené bylo riziko infekce, mechanického poškození, okluze a riziko spojené s nevhodně provedenou edukací ošetřujících osob. Výsledky analýzy ukázaly, že se jedná o vážná rizika, která mohou mít pro závažně nemocné děti fatální následky. Výstupem diplomové práce je brožura, ve které je popsán s pomocí obrázků postup péče o PICC, PORT a Broviac, Hickmann.

Abstract:

This thesis focuses on the care of invasive venous access in a pediatric patient in palliative care. The theoretical part describes the development of palliative care in the world and in the Czech Republic, especially in pediatric palliative care. It also describes venous accesses that are most commonly used in children requiring palliative care and the risks associated with their use. The empirical part presents the results of the literature search and brainstorming. The aim of the research was to use the Failure Mode and Effects Analysis (FMEA) method to identify the most common risks that may occur when treating invasive venous access in a pediatric patient receiving palliative care. The work focused on the risks of tunneled central venous catheters and port. The risk of infection, mechanical damage, occlusion and the risk associated with inadequate caregiver education were assessed. The results of the analysis showed that these are serious risks that can have fatal consequences for seriously ill children. The output of the thesis is a booklet that describes, with the help of pictures, the care procedure for PICC, PORT and Broviac, Hickmann.
