Vyšetření spermií nositelů balancované translokace pomocí fluorescenční in situ hybridizace

The examination of spermatozoa from carriers of balanced translocations using the fluorescence in situ hybridization method

Abstract:

Vrozené balancované strukturní aberace chromozómů se vyskytují asi u 0,3 % lidské populace. Přítomnost balancované translokace v karyotypu většinou nevede k závažným fenotypovým projevům. Problémem však zůstává různý stupeň omezení fertility a zvýšené riziko narození postiženého potomka s nebalancovaným karyotypem. Metodou mnohobarevné fluorescenční in situ hybridizace s centromerickými, telomerickými a lokus specifickými sondami pro chromozomy podílející se na translokaci bylo vyšetřeno 7 nositelů Robertsonské translokace a 22 nositelů reciproké translokace. Při segregačních studiích spermií nositelů balancovaných translokací se používají různé kombinace DNA sond pro jednotlivé chromozomy. Zjištěná frekvence chromozomálně abnormálních spermií u nositelů Robertsonské translokace se pohybuje v rozmezí 5,8 % až 23,5 % a u nositelů reciproké translokace v rozmezí 40,1 % až 70,3 %. Riziko vzniku nebalancovaných embryí u mužů s reciprokými translokacemi je významně vyšší než u nositelů Robertsonských translokací. Frekvence jednotlivých typů abnormální segregace u reciprokých translokací závisí na chromozomech zahrnutých v translokaci, na pozici zlomů a jsou ovlivněny přítomností intersticiálních chiasmat. Interfázní fluorescenční in situ hybridizace na dekondenzovaných jádrech spermií nositelů translokace se ukázala jako hodnotný nástroj pro segregační studie u nositelů balancovaných translokací. V praxi je možné získané výsledky využít v genetickém poradenství a reprodukční medicíně. Využití výsledků vyšetření segregace translokovaných chromozomů ve spermiích k predikci úspěšnosti celého cyklu asistované reprodukce, volbě intracytoplasmatické injekce spermie do oocytu a preimplantační genetické diagnostiky, či použití spermatu dárce.

Abstract:

Congenital balanced structural aberrations of chromosomes occur in about 0.3% of human population. Presence of a balanced translocation in karyotype does not usually result in serious phenotypic abnormalities. Their presence in karyotype often causes a significant reduction in fertility and increased risk of birth of an affected offspring. Using the multicolor fluorescence in situ hybridization with centromeric, telomeric and locus specific probes for chromosomes involved in translocation, 7 Robertsonian translocation carriers and 22 reciprocal translocation carriers were examined. In the segregation studies of sperm from balanced translocation carriers, various combinations of DNA probes are used for respective chromosomes. Frequencies of chromosomally abnormal spermatozoa in Robertsonian translocation carriers and reciprocal translocation carriers range between 5,8 % – 23,5 % and between 40,1 % - 70,3 %, respectively. Frequency of non-balanced spermatozoa is significantly higher in men with reciprocal translocations than in men who are carriers of Robertsonian translocations. Frequency of different types of abnormal segregation in reciprocal translocations depends on the chromosomes involved in translocation and on the position of breaks, and is affected by presence of interstitial chiasms. The interphase fluorescence in situ hybridization in decondensed nuclei of sperm from translocation carriers showed to be a valuable tool for segregation studies in balanced translocation carriers. Under practical conditions, the obtained results can be used in genetic consulting and reproductive medicine. The results of segregation studies of translocated sperm chromosomes can be used for prediction of outcome in the whole cycle of assisted reproduction, option for intracytoplasmic sperm injection into an oocyte and preimplantation genetic diagnosis or the use of a donor’s sperm.
