Epidermolysis bullosa congenita - genotype and phenotype corelation in patients from EB Centre CR

Abstract:

Úvod. Epidermolysis bullosa congenita (EB) je vzácné dědičné onemocnění vedoucí ke zvýšené fragilitě kůže. EB je způsobena mutacemi v genech převážně pro strukturální proteiny kůže, které zajišťují mechanickou odolnost epidermis a její ukotvení k hlubším strukturám dermis. V roce 2014 byla publikována současná klasifikace, která rozlišuje 40 různých forem EB lišících se klinickými projevy i prognózou. Klinická variabilita EB je dána rozdílnými funkcemi postižených proteinů. Pro stanovení správné diagnózy a porozumění klinickému průběhu onemocnění je důležité poznání patogenních mutací asociovaných genů a pochopení, jak ovlivňují syntézu a funkci kódovaných proteinů. Metody. V první části práce byla provedena detailní rekapitulace současných znalostí o vlivu různých typů mutací genů na fenotyp EB. V další části byla analyzována data pacientů EB Centra ČR. V souboru pacientů bylo zkoumáno zastoupení jednotlivých typů mutací. K analýze byla použita deskriptivní statistika a výsledky byly srovnány s literárními zdroji. Na základě dostupných dat byla spočítána prevalence a incidence EB v ČR. Výsledky. K nejzávažnějším formám EB vedou převážně recesivně děděné mutace, které způsobují vznik předčasného terminačního kodonu a ukončení syntézy kódovaného proteinu. Předčasný terminační kodon je nejčastěji způsoben nesmyslnými mutaci a mutacemi měnící sestřih. V EB Centru ČR je evidováno 295 pacientů, z toho 165 pacientů s EB simplex, 117 s dystrofickou EB a 13 s junkční EB. Bodová prevalence k prosinci 2015 byla 24.6 pacientů s EB na jeden milion obyvatel. Incidence EB v České republice je 7 případů na 100,000 živě narozených dětí. V EB Centru ČR bylo diagnostikováno několik vzácných typů EB. U 79% pacientů EB Centra ČR byla provedena DNA molekulární analýza, která vedla k objevení 40 nových, dříve nepopsaných mutací. Závěr. EB je heterogenní skupina dědičných onemocnění. K onemocnění vedou převážně mutace strukturálních proteinů epidermis a dermoepidermální junkce. Různé typy mutací odlišným způsobem ovlivňují syntézu kódovaných bílkovin a tím klinickou variabilitu EB. Péče o pacienty s EB vyžaduje multioborový přístup, centralizaci pacientů v EB Centru a spolupráci s pacientskou organizací DEBRA.

Abstract:

Background. Epidermolysis bullosa congenita (EB) is a rare, inherited disease leading to increased fragility of the skin. The present classification distinguishing 40 different forms of EB with different clinical manifestation and prognosis was published in 2014. Variability of EB is driven by the variability of protein malfunctions. Recognition of mutations leading to protein alteration substantialy contributes to correct diagnosis and helps understand a clinical course of the disease. Methods. In the first part of the thesis a detailed review of how different mutations influence the phenotype of EB is presented. In the next part, a data analysis of patients from the EB Centre Czech Republic was performed. Descriptive and epidemiological summaries were given for group of patients with different types of mutations. Clinical course and its association with genetic backround of pateints with EB were discussed. Results. EB is mainly related to impaired function of structural proteins, which ensure anchoring of the epidermis to the dermis. Recessively inherited mutations, which lead to premature termination codon, cause the most serious forms of EB due to protein synthesis termination. Nonsense and splicing mutations lead to premature termination codon in most cases. In the EB Centre CR 295 patients with EB is registered to date (165 with EB simplex, 117 with dystrophic EB, 13 with junctional EB). EB point prevalence (December 2015) was 24.6 patients per million inhabitants. The incidence of EB in the Czech Republic was 7 cases per 100,000 live births. Patients with several rare types of EB are also registered in the EB Centre CR. DNA molecular analysis was performed in 79% of patients with EB and 40 novel mutations were discovered. Conclusions. EB is a heterogeneous group of inherited diseases. The disease is predominantly caused by mutations of structural proteins of the epidermis and the dermo-epidermal junction. Different type of mutations resulting in altered proteosyntesis are responsible for different clinical courses of the disease. Medical care of patients with EB requires a multidisciplinary approach and leads to the establishment of EB Centres and patient organizations DEBRA.
