Multivariate statistical methods as a tool to analyze hospital inpatient diversity

Abstract:

Ústředním tématem této bakalářské práce je diverzita neboli popis struktury hospitalizovaných pacientů. V úvodu práce jsou přiblíženy systémy, které slouží k roztřídění případů do skupin. Klasifikační systém IR-DRG aktuálně využívaný v České republice mimo jiné k úhradě akutní lůžkové péče trpí řadou nedostatků, zejména nedostatečnou homogenitou jednotlivých kategorií. Cílem práce je vytvoření nových skupin případů pro vybranou onkologickou diagnózu a to na základě podobnosti vykázaných výkonů. Na datový soubor pacientů hospitalizovaných pro zhoubný novotvar mozku (z databáze I-COP) byly aplikovány metody vícerozměrné statistické analýzy popsané v teoretické části práce. Nejprve byly uplatněny dvě varianty redukce dimenze: analýza hlavních komponent (PCA) a analýza hlavních koordinát (PCoA) a následně byly vytvořeny shluky metodou k-průměrů. Pro výběr vhodného počtu shluků bylo využito kritérium průměrné siluety, vzhledem k nízkým hodnotám pak i dílčí průměry shluků a podíl kladných siluet pro jednotlivá rozdělení. Byly vytvořeny 2 varianty skupin: 9 pro PCA a 11 pro PCoA. Na základě jejich interpretace byly za uspokojivější označeny výsledky z PCA.

Abstract:

The main focus of this bachelor thesis is diversity, or in other words, description of an inpatient structure. The initial part of the thesis describes systems used for dividing cases into groups. The classification system IR-DRG, which is currently used in the Czech Republic among others for payment of acute inpatient care, suffers from series of flaws. Particularly, separate categories do not achieve sufficient homogeneity. The aim of this thesis is to create new groups of cases for selected oncological diagnosis based on similarity of underwent procedures. Methods of multivariate statistical analysis described in the theoretical part were applied on the dataset of inpatients with malignant neoplasm of brain diagnosis (from the I-COP database). Two variations of dimension reduction were used: principal component analysis (PCA) and principal coordinate analysis (PCoA) followed by forming groups using k-means clustering. The silhouette criterion was used to select the appropriate number of clusters, due to low mean values also partial means for clusters and ratio of positive silhouettes for individual partitioning were computed. Two alternatives of groups were created: 9 for PCA and 11 for PCoA. Based on their interpretations, the results of PCA variant were considered more satisfying.
