Biliary Manometric Perfusion Test in the Evaluation of Percutaneous Treatment of Benign Biliary Strictures

Abstract:

Benigní stenózy žlučových cest jsou většinou léčeny chirurgicky nebo endoskopicky. Pokud chirurgická či endoskopická léčba nelže nebo není proveditelná je indikovaná léčba perkutánní. Perkutánní léčba zahrnuje iniciální drenáž, dilataci stenózy balónkovými dilatačními katétry a dlouhodobou zevně vnitřní drenáž. Účinnost perkutánní léčby benigních stenóz žlučových cest balónkovou dilatací je obvykle hodnocena klinickým testem. Nově bylo pro objektivní zhodnocení úspěšnosti léčby navrženo měření tlaku ve žlučových cestách (biliární manometrický perfúzní test). Cíl prace je posoudit proveditelnost biliárního manometrického perfúzního testu v hodnocení radiologické perkutánní léčby benigních stenóz žlučových cest, zhodnotit výtěžnost biliárního manometrického perfúzního testu v hodnocení léčby v souvislosti s dlouhodobou průchodností po perkutánní léčbě, srovnat výtěžnost biliárního manometrického perfúzního testu s v současné době prováděnými klinickými testy v hodnocení perkutánní balónkové dilatační léčby benigních stenóz žlučových cest

Abstract:

Benign biliary strictures are usually treated surgically or endoscopically. When the surgical repair or the endoscopic treatment are not feasible or fails then radiological percutaneous dilatation treatment is indicated. Percutaneous treatment includes initial drainage and dilatation of stricture by balloon angioplasty catheters of different diameters and long term external-internal drainage. Benign biliary strictures treated percutaneously by balloon dilatation treatment are usually evaluated for treatment success and adequacy by clinical test. For the objective evaluation of treatment success, measurement of pressure in the biliary tree (biliary manometric perfusion test) has been proposed. The aim of the study is to assess the feasibility of to assess the feasibility of biliary manometric perfusion test in the evaluation of radiological percutaneous treatment of benign biliary strictures, to evaluate the success of biliary manometric perfusion test in treatment evaluation in relation to the long term patency after the percutaneous treatment,to compare the efficacy of biliary manometric perfusion test with the present clinical test in the evaluation of percutaneous balloon dilatation treatment of benign biliary strictures
