Life Situation of Family Caregivers and Social Work

Abstract:

Disertační práce se věnuje tématu sociální práce na obecních úřadech zaměřené na životní situace spojené s poskytováním dlouhodobé rodinné péče. Cílem práce je na základě teoretické i vlastní empirické poznatkové báze porozumět tomu, jak sociální pracovníci obecních úřadů reagují na životní situaci rodinných pečujících. V teoretické části práce jsou shrnuty relevantní zahraniční a tuzemské poznatky týkající se zejména specifik dlouhodobé rodinné péče, rizik a nákladů spojených s poskytováním rodinné péče, dynamiky rodinné péče a také typů podpory dlouhodobé rodinné péče a sociálně politických nástrojů a opatření určených k pomoci a podpoře rodinných pečujících. Zvláštní pozornost je věnována možnostem sociální práce jako pomáhajícího oboru v oblasti pomoci a podpory rodinným pečujícím. Epistemologickým východiskem studie a tedy rámcem pro volbu metody, analytických postupů a interpretace dat je sociální konstrukcionismus. Výzkumná otázka se ptá se na reagování sociálních pracovníků na životní situaci těch, s nimiž se ocitají v interakci, v tomto případě rodinných pečujících. To je pohled konsistentní s východisky sociálního konstrukcionismu, který předpokládá určitý způsob poznávání, způsob přemýšlení o lidech, událostech a problémech, který zároveň vede i jednání aktérů. Výrazem „reagování na životní situaci rodinných pečujících“ je pak označován proces, ve kterém sociální pracovníci na základě své kognitivní výbavy, v kontextu své profesní socializace a pojetí sociální práce a pod vlivem své pracovní situace konstruují obraz okolností života, na které je podle nich relevantní v rámci výkonu jimi interpretované náplně vlastní práce v organizaci působit. Na základě toho působí nebo zvažují, že budou působit na rodinné pečující jednáním, které je podle za daných okolností adekvátní. Pro zajištění dostatečně robustního datového korpusu byl zvolen smíšený kvalitativní a kvantitativní výzkumný design. V kvalitativním šetření byli jednotkou zjišťování sociální a vedoucí pracovníci sociálních odborů/oddělení obcí s rozšířenou působností zařazených do výzkumu záměrným výběrem. Kvantitativní šetření navazovalo na kvalitativní výzkum. Bylo orientováno na sociální pracovníky obcí s rozšířenou působností a bylo pojato jako vyčerpávající. Klasický kvalitativně – kvantitativního design byl doplněn specifickou případovou studií. Dominantním zdrojem dat byl kvalitativní výzkum, ale prezentace výstupů z analýz všech tří datových setů umožnila ucelenější porozumění zkoumanému problému. Metodou analýzy kvalitativních dat byla tematická analýza. Jako postup přiměřený induktivně-deduktivnímu charakteru analýzy bylo zvoleno otevřené axiální kódování. Výsledky výzkumu potvrzují, že rodinní pečující nejsou etablovanou cílovou skupinou sociálních pracovníků. Cíle sociální práce zaměřené na rodinné pečující byly vymezovány neurčitě či se vůbec nestanovují; popisovaná nabízená či poskytovaná pomoc je nespecifická a odvozená ze zkušeností s jiným cílovými skupinami, za nimiž jsou pečující „schovaní“. Zůstávají tedy sociálním pracovníkům skryti za „tradičními“ cílovými skupinami sociální práce. Problémy a potřeby rodinných pečujících sociálním pracovníkům snadno splývají s problémy a potřebami příjemců péče. Výchozí premisou stávající praxe sociální práce zaměřené na rodinné pečující je očekávání automatické schopnosti a možnosti rodinných příslušníků pečovat. Tato premisa je na straně sociálních pracovníků spíše implicitní a neuvědomovaná. Určuje ale převažující pojetí sociální práce z

aměřené na životní situace rodinných pečujících. Výrazným jevem je zejména přehlížení potřeb pečujících či velmi úzké vymezení těchto potřeb. V potaz jsou brány především ty potřeby pečujících, jejichž naplnění je nezbytné pro zachování schopnosti a možnost pečovat či v péči pokračovat. Faktické přehlížení rodinného pečujícího odpovídá pojetí rodinných pečujících označovanému „pečující jako zdroj“ a je ve výkonu sociální práce často doprovázeno chybějícím diferencováním mezi potřebami pečujícího a pečovaného. V jednotlivých případech, zejména pokud se rodinný pečující na sociálního pracovníka obecního úřadu sám obrátí, je pravděpodobné, že se mu určité pomoci dostane. Tato pomoc ale dle získaných empirických poznatků reaguje především na dílčí potíže bez kontextu celé životní situace rodinného pečujícího a jde nejčastěji o jednorázovou pomoc formou sociálního poradenství. Sociální pracovníci také často poukazovali na limity pomoci rodinným pečujícím, jako je nedostatek zdrojů pomoci v lokalitě, nedostupnost sociálních služeb, neochota pečujících pomoc přijmout atd. Z kvantitativních dat se ale zároveň jeví, že sociální pracovníci jsou poměrně pasivní, co se týče mapování zdrojů pomoci rodinným pečujícím dostupných v jejich lokalitě. Implicitní předpoklad, že dokud si nikdo nestěžuje a nikdo na nic neupozorňuje a výslovně nic nežádá, nevzniká podklad pro činnost, je doprovázen pasivitou v tom, že sociální pracovníci většinou čekají, zda pečující sami přijdou, případně spoléhají na podněty jiných subjektů či pomáhajících pracovníků. Rovněž specifických snah cíleně dostat k pečujícím informace o nabízené pomoci ze strany sociální práce na obecních úřadech bylo v rozhovorech popsáno poměrně málo. Lze to chápat jako další ze symptomů v teoretické části studie vysvětleného tzv. nepodporovaného familiarismu v oblasti dlouhodobé péče.

Abstract:

The dissertation thesis is focused on the topic of social work dealing with long-term family care life situations at municipal authorities. The primary purpose of the thesis is to capture how municipal social workers respond to the life situation of family caregivers. The theoretical part of the thesis summarizes the relevant knowledge concerning mainly the specifics of long-term family care, the risks and costs associated with family care provision, long-term care dynamics, as well as the kinds of help and support that are provided by family caregivers and social policy tools and measures designed to help and support family caregivers. Special attention is paid to exploring the possibilities of social work as a helping profession to be of assistance and support to family caregivers. Social constructionism is the epistemological basis of the study and thus the framework for the choice of method, analytical procedures and data interpretation. The main research question asks how municipal social workers respond to the life situation of family caregivers. Such a question is consistent with the social constructionism views of how the knowledge is created, how social actors, in particular, and social circumstances negotiate and create meaning through communication and how they think and act on the basies of this created knowledge and understanding. The term "responding to the life situation of family carers" refers to the process by which social workers, on the basies of their cognitive faculties and in the context of their professional socialization, including their concept of social work and under the conditions of their working situation, construct an idea of the possible and appropriate actions within the given circumstances. A mixed qualitative and quantitative research design was chosen to gather a sufficiently robust data corpus. In the qualitative survey, social workers and heads of social departments at municipality offices were selected as the research participants through purposive expert sampling. The quantitative research followed the qualitative research and was focused on all the municipal social workers. The qualitative - quantitative design model was complemented by a specific case study. The qualitative research was the dominant data source; however, the use of all three data sets enables a more comprehensive understanding of the research topic. Thematic analysis was applied to the qualitative data analysis method. An open axial coding was chosen as a procedure that was appropriate to the inductive-deductive nature of the analysis. The research results confirm that family caregivers are not an established social work target group. The goals of social work with family caregivers have been set imprecisely or are entirely missing; the work they offer or provide to family caregivers is only vaguely described by social workers, and it is obvious that it is derived from experience with other, more “traditional" social work target groups behind which caregivers are "hidden". Thus from the social workers point of view, the problems and needs of family caregivers blend easily with those of the care receivers. The default, and rather implicit and unreflected, premise the social workers often proceed from in their practice of social work aimed at family caregivers is the belief that family members are automatically able and competent at care provision. This determines the prevailing social work practice focused on the life situations of family carers. Overlooking the needs of caregivers or the very narrow perception on their needs has thus resulted in a significant phenomenon. Through the study, the needs of caregivers are taken into account, particularly their fulfillment, which is essential to keep them going, to maintain their ability to care. d by the lack of differentiation between the needs of caregiver and care receiver. In individual cases, especially if the family caregivers turn to the municipal office worker themselves, it is likely that they will receive some help. However, according to gained empirical findings, this help responds almost entirely to only a part of the problems and difficulties. It does not take into consideration the context of the whole life situation of the family caregiver and is most often a one-time form of help provided through social counselling. Social workers have also often pointed out the limits of help and support for the family caregivers, such as the lack of local resources and services, reluctance of caregivers to accept help, etc. At the same time, quantitative data seem to indicate that social workers are relatively passive in mapping those resources at their disposal within their municipality. The implicit assumption is that until somebody complains or draws attention to something, or explicitly asks for something, there is no basis for the social worker to take action. Simultaneously, this approach is accompanied by the rather passive expectation of social workers that family caregivers will contact them on their own accord, or will rely on the initiatives of the helping agencies/workers. There have also been only a few references to the purposeful effort to obtain specific information about the support and help offered to caregivers by social workers at the municipal authorities. This can be seen as one of the main symptoms of the unsupported familialism in long-term care, which is mentioned in the theoretical part of the study.
