Awareness about nosocomial infections in patients and lay population

Abstract:

I přes významný pokrok medicíny v posledním decéniu zůstávají nozokomiální nákazy stále významnou příčinou morbidity a mortality hospitalizovaných pacientů a jejich incidence nezaznamenává klesající trend. Některá pracoviště a zdravotnická zařízení si uvědomují, že sledování výskytu a příčin nozokomiálních nákaz je nejenom zrcadlem jejich kvality, ale že aplikace cílených preventivních opatření podmíněných specifickými místními faktory na základě vlastní surveillance přináší lepší prognózu pro hospitalizované pacienty i šetření finančních nákladů na léčbu. Není tomu tak však ve všech případech, pořád se u nás vyskytují nemocnice hlásící téměř nulový výskyt nozokomiálních nákaz a představují tak riziko pro pacienty kvůli neexistující zpětné vazbě. Hnacím motorem k tomu, aby nemocnice důsledně hlásily výskyt nemocničních nákaz a nepodceňovaly jejich prevenci, může být i informovanost o problematice NN u samotných pacientů, která může přispět k zlepšení kvality zdravotní péče a k poklesu incidence nemocničních nákaz. Cílem práce bylo zjistit úroveň informovanosti laické populace ? pacientů o nemocničních nákazách a jejich prevenci. Pro zjištění úrovně informovanosti byl zvolen dotazníkový průzkum u 60 respondentů různých věkových skupin. Z dotazníkového šetření vyplývá, že 67 % respondentů ví, co to jsou nemocniční nákazy, zbývající dotazovaní se s tímto pojmem buď nesetkali, anebo o nemocničních nákazách slyšeli, ale neví, o co se jedná. Celkem 4 osoby uvedly výskyt nemocniční nákazy u nich samých či u někoho z jejich okolí. Velká část laické populace však není o této problematice dostatečně informována a nemusí si být vědoma, že nemocniční nákazou onemocněla. Znalost MRSA, jako jednoho z nejvýznamnějších nozokomiálních problémů dnešní doby, je u laické populace prakticky nulová, a to i přestože tento pojem zaznamenává v posledních letech velkou pozornost odborníků i médií. Výsledky ukázaly i na neznalost minimální hranice výskytu nemocničních nákaz. Většina respondentů by se nechala hospitalizovat v nemocnici, která hlásí výskyt nemocničních nákaz blížící se co nejvíce k 0, i když minimální hranice incidence je reálně na úrovni 6 - 8 %. Z výsledků vyplývá, že až 75 % respondentů je přesvědčeno, že informovanost pacienta může zlepšit kvalitu zdravotní péče, protože jim dává větší možnost při výběru zdravotnického zařízení, možnost identifikace rizika hospitalizace, kontroly dodržování preventivních opatření i lepší spolupráci se zdravotníky. Důležité je však zabezpečit adekvátní formu edukace laické populace v této oblasti.

Abstract:

Despite significant progress in medicine in the last decade, nosocomial infections remain a significant cause of morbidity and mortality of hospitalized patients and their incidence is not decreasing. Some healthcare facilities are aware that monitoring of the incidence and causes of nosocomial infections is not only a mirror of their quality, but that application of targeted preventive measures influenced by on specific local factors on the basis of their own surveillance has improved the prognosis for hospitalized patients and treatment costs saving. However that is not the case all the time. There are hospitals in the Czech Republic reporting almost zero occurrence of nosocomial infections and the present a risk to patients due to non-existing feedback. A driving force for reporting and no underestimating of nosocomial infections can be patients education about nosocomial infections, which can contribute to improving the quality of health care and lead to the decrease of hospital infections incidence. The aim of this work was to determine the level of the general public awareness of hospital infections and their prevention by means of a survey in a group of 60 respondents of various age. The survey shows that 67 % of the respondents know what hospital infections mean, the remaining respondents either have not met this term or they have heard of it but they do not know the exact meaning of it. The total of 4 people reported harring personal experience with hospital infection or knowing somebody with this experience. Large part of the lay public is not sufficiently informed of this issue and these people might not be aware of having hospital infection. General knowledge of MRSA as one of the most important nosocomial problems nowadays is practically zero despite the fact, that this term has been drawing a lot of experts and media attention in the past years. The results also demonstrated the lack of knowledge about minimum limit of hospital infections incidence. Most respondents would choose to be hospitalized in the hospital, which reported the incidence of hospital infections close to 0, although the minimum limit of the real incidence is at the level of 6 - 8 %. We can see from the results that up to 75 % of respondents believe that patient awareness can improve the health care quality, because it gives them the greater possibility for the selection of better healthcare facility, the possibility to estimate the risk of hospitalization, control of preventive measures observance and better cooperation with health professionals. It is important to ensure an adequate form of the lay public education in this area.
