Psychotherapy of trauma-related dissociation – a systematic review

Abstract:

Teoretické pozadí: Disociace v důsledku traumatu je jedním z mnoha důsledků pře-žití traumatické události. V důsledku disociace se cítíme odpojení od vlastního těla, vzpomínek nebo identity. V kontextu traumatu je disociace obranným mechanismem proti přetížení silnými emocemi. Cíl: Cílem této práce bylo vytvořit systematický přehled psychoterapeutických pří-stupů a technik, které pracují s disociací, která souvisí s traumatem. Metoda: Během systematického přehledu bylo identifikováno 1303 studií. Výběr studií proběhl 21. března 2024. Některé studie byly vyřazeny pomocí softwaru Ray-yan a jiné byly identifikovány na základě analýzy názvů a abstraktů. Kritéria pro za-řazení do přehledu nakonec splnilo 35 studií. Výsledky: Naše zjištění identifikovala několik psychoterapeutických přístupů a technik, které jsou vhodné pro práci s disociací související s traumatem. Přístupy, jako je fázově orientovaný přístup, představují základní přístup k léčbe traumatizo-vaných jedinců. Zahrnuje takové techniky, jako je psychoedukace nebo racionální terapie. Ke zpracování samotného traumatu lze využít EMDR, CBT nebo techniky za-měřené na práci s tělem při prožívání traumatu. Závěr: Výsledkem práce je přehled používaných psychoterapeutických přístupů a technik při práci s disociací související s traumatem. Re-view shrnuje studie od roku 1994. Podle našeho názoru by se budoucí výzkum měl zaměřit na prevenci traumatu a práci s přístupem k léčbě traumatu „tady a teď“.

Abstract:

Background: Trauma-related dissociation is one of the many consequences of sur-viving a traumatic event. As a result of dissociation, we feel disconnected from our own body, memories, or identity. In the context of trauma, dissociation is a defense mechanism against being overwhelmed by strong emotions. Objective: The aim of this work was to create a systematic review of psychothera-peutic approaches and techniques that work with trauma-related dissociation. Method: 1303 studies were identified during the systematic review. The selection of studies took place in March 21, 2024. Some studies were excluded using Rayyan software and others were identified based on the analysis of titles and abstracts. Fi-nally, 35 studies met the criteria for inclusion in the review. Results: Our findings identified several psychotherapeutic approaches and tech-niques suitable for working with trauma-related dissociation. Approaches such as the phase-oriented approach represent the fundamental principle of treating trauma-tized individuals. It also includes techniques such as psychoeducation or rational therapy. EMDR, CBT, or techniques aimed at working with the body when experienc-ing trauma can be used to process the trauma itself. Conclusion: The main contributiont of this work is an overview of psychotherapeu-tic approaches and techniques used in working with trauma-related dissociation. The review summarizes studies since 1994. In our opinion, future research should focus on trauma prevention and utilizing a "here and now" approach to trauma treatment.
