Tourniquets Effectiveness in Traumatic Bleeding

Abstract:

The thesis evaluates tourniquets effectiveness usable in trauma bleeding situations. Aim of the thesis is to compare and evaluate selected tourniquet types: Combat Application Tourniquet, Stretch Wrap & Tuck Tourniquet and the basic Esmarch bandage. Theoretical part includes knowledge about physiology of haemorrhage and haemostasis processes. It also consists in options of trauma – haemorrhage stage therapy at the Pre – Hospital care and in Pre-Hospital Trauma Life Support guidelines for treating trauma patient by Emergency Medical Services. The practical part of the work carries out the quantity research of tourniquet effectiveness for which has been used Doppler ultrasonography method. Empiric part outcome is the protocol of recorded data which have been got by Tourniquet applied to 25 respondents. Esmarch bandage has not been able to make required vessel occlusion pressure which is a surprise according to all that it is the mandatory first aid kit equipment. After the research results as compared to professional articles there are practically beneficial assignments that Combat Application Tourniqet can generate vessel occlusion in less time period than Stretch Wrap & Tuck Tourniqet. Application of Stretch Wrap & Tuck Tourniquet by Emergency Services to casualties suffering from massive haemorrhage considered as advantage because of causing less pain which is great to use especially for pediatric patients.

Abstract:

Bakalárska práca sa zaoberá hodnotením efektivity zaškrcovacích pomôcok použiteľných pri traumatickom krvácaní. Cieľom práce je porovnať a zhodnotiť vybrané zaškrcovacie pomôcky: Combat Application Tourniquet, Stretch Wrap and Tuck Tourniquet a Esmarchovo zaškrcovadlo. Teoretická časť bakalárskej práce zahrňuje teoretické poznatky o fyziológií krvácania a procesoch hemostázy, popisuje možnosti liečby hemoragicko-traumatického stavu v prednemocničnej starostlivosti a všeobecným postupom posádok záchrannej služby u traumatického pacienta na základe princípov Pre-Hospital Trauma Life Support. Praktická časť práce sa venuje kvantitatívnemu výskumnému hodnoteniu zaškrcovacích pomôcok. Pre výskumné meranie oklúznych časov zaškrcovacích pomôcok bola využitá metóda dopplerovskej ultrasonografie. Výstupom empirickej časti bol protokol so zaznamenanými údajmi získanými pri aplikácii zaškrcovacích pomôcok na 25 dobrovoľných respondentoch. Esmarchovo zaškrcovadlo ani v jednom z našich meraní vykonávanom na respondentoch nevytvorilo dostatočný oklúzny tlak cievneho povodia, čo je prekvapujúce zistenie vzhľadom k tomu, že je povinnou výbavou autolekárničiek. Z výsledkov meraní, ktoré boli konfrontované aj s odbornými článkami, sú pre prax prínosné zistenia, že Combat Application Tourniquet vytvorí oklúziu cievneho povodia za kratší čas ako Stretch Wrap and Tuck Tourniquet. Výhodou Stretch Wrap and Tuck Tourniquet je jeho aplikácia záchrannými zložkami poraneným osobám s masívnym život ohrozujúcim krvácaním pre jeho nižšiu bolestivosť, ktorá je vítaná obzvlášť u pediatrických pacientoch.
