Comparison between Video Head Impulse Test and Caloric test for the purpose of vestibular system examination

Abstract:

Úvod: Kalorické vyšetření bylo donedávna jedinou objektivní diagnostickou metodou, která byla schopna izolovaně vyšetřit funkci laterálních polokruhovitých kanálků, které se podílejí na udržování rovnováhy. Od roku 2014 se v České republice začala nově vyšetřovat funkce všech polokruhovitých kanálků pomocí Video Head Impulse Testu (VHIT). Vědecké studie doposud ukazují, že se výsledky obou diagnostických metod často neshodují. Cíl: Cílem studie bylo porovnat výsledky kalorického vyšetření a VHIT u souboru pacientů s periferním vestibulárním syndromem na specializovaném pracovišti ambulantního typu. Metodika: Soubor pacientů byl vyšetřován od července 2016 do srpna 2017. Vyšetřili jsme 523 osob s poruchou rovnováhy. Kompletní otoneurologické vyšetření absolvovalo 98 osob s podezřením na možné periferní postižení vestibulárního systému. Na základě vylučovacích kritérií byli do studie zařazeni pacienti (n=32; 24 žen, 8 mužů; průměrný věk 49,4 let) s jednostranným periferním vestibulárním syndromem a pozitivním nálezem kalorického vyšetření nebo VHIT. Statisticky byla hodnocena shoda výsledků obou diagnostických metod. Dále byl zkoumán vztah časového průběhu onemocnění a výsledků VHIT. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Pozitivní výsledek kalorického vyšetření byl nalezen u 90 % pacientů, zatímco pozitivita VHIT byla u 50 % pacientů souboru. U 13 probandů se pozitivní nález shodoval u obou metod, u 16 bylo kalorické vyšetření pozitivní a VHIT negativní, u 3 probandů bylo kalorické vyšetření negativní a VHIT pozitivní. Na základě těchto hodnot je shoda výsledků obou diagnostických metod 40, 6 %. Tento rozdíl výsledků je statisticky významný. Dle VHIT byla nalezena u 13 pacientů kromě poruchy horizontálních kanálků také porucha i v některém z vertikálních kanálků: 4x pravý přední, 4x levý zadní, 3x levý přední, 2x pravý zadní. Rozdělením souboru do skupin podle časového průběhu a spouštěcích faktoru potíží, byl VHIT pozitivní u akutního spontánního vertiga v 62,5 % případů. U epizodických spontánních a chronických potíží byl VHIT ze 70 % negativní. Zde nebyl nalezen statisticky významný rozdíl. Závěr: Výsledky kalorického vyšetření a VHIT se v diagnostice vestibulárních poruch často neshodují. Při diagnostice vestibulárních poruch je třeba vzít v potaz samotnou patofyziologickou podstatu onemocnění, která se u periferních poruch liší a může být příčinou diskrepance výsledků obou metod. Obdobně mechanismus obou vyšetření není identický a může být důvodem rozdílných výsledků. VHIT se jeví jako další metoda vyšetření vestibulárního aparátu, která podává nové informace o stavu rovnovážného systému a doplňuje komplexní pohled na pacienta se závratí. VHIT je v detekci narušené funkce vestibulo-okulárního reflexu polokruhovitých kanálků více specifickým testem než senzitivním. V případě pozitivního výsledku VHIT není nutné doplňovat kalorické vyšetření. V opačném případě je potřeba zvážit kalorizaci k potvrzení či vyloučení nízkofrekvenční poruchy VOR. Časový průběh onemocnění může být prediktorem výsledku VHIT. Lokalizace porušené funkce polokruhovitých kanálků dle VHIT dává informaci pro selektivní vestibulární rehabilitaci.

Abstract:

Introduction: Caloric stimulation testing was until recently the only method for diagnosing the function of the lateral semi-circular canals responsible for maintaining balance. Since 2014, the Video Head Impulse Test (VHIT) is being used in the Czech Republic to investigate the function of all semi-circular canals. Studies so far have shown that the results of these two methods are often not in agreement. Aim: Our objective was to compare the results of vestibular caloric stimulation and VHIT in a cohort of patients with peripheral vestibular syndrome at a specialized outpatient clinic. Method: The cohort was tested between July 2016 and August 2017. Diagnosis was performed on 523 patients with balance disorders. 98 of them with suspected peripheral vestibular syndrome underwent comprehensive otoneurological testing. Exclusion criteria were applied to select patients with unilateral peripheral vestibular syndrome (n=32; 24 females and 8 males; mean age 49.4 years) and a positive result with either vestibular caloric stimulation or VHIT. Agreement between the two methods was statistically evaluated at a significance level of 5 %. We also studied the association between VHIT test results and the progress of the syndrome over time. Results: Caloric stimulation was positive in 90 % of the cohort while VHIT was positive only in 50 % of the patients. 13 patients were positive in both tests, caloric stimulation was positive and VHIT negative in 16 patients, while in 3 patients caloric stimulation was negative and VHIT positive. The agreement between the methods is thus 40.6 %. This difference is statistically significant. In 13 patients, apart from lateral canal disorders, VHIT revealed disorders in one of the vertical canals: the front right in 4, the rear right in 2, the front left in 3 and the rear left in 4. When sorted into subgroups based on the time course and triggering factor, VHIT was positive for acute spontaneous vertigo in 62.5 % of cases. VHIT was negative for 70 % of chronic and spontaneous episodic complaints. There was no statistically significant difference among these cases. Conclusion: During diagnosis of vestibular disorders, the results of caloric stimulation and VHIT often do not agree. The diagnosis of peripheral vestibular disorders should take the pathophysiological basis of the disease into account, as this can differ in peripheral disorders, and can contribute to the discrepancy between the two methods. VHIT can be used to diagnose disorders in the vestibular system, and adds to the information about the status of the vestibular system, providing for a comprehensive view of a patient with vertigo. VHIT is more of a specific testing tool for diagnosing an abnormal vestibuloocular reflex in the semi-circular canals, but it is not a sensitive tool. When VHIT is positive caloric stimulation is no longer necessary, but when it is negative, caloric stimulation must be considered to either confirm or eliminate low-frequency abnormalities in the vestibuloocular reflex. The time course of the disorder can be a predictor of the VHIT test result. VHIT can be used to localize the disorder in the semi-circular canals and this information can be used for selective vestibular rehabilitation
