Implementation of preconception carrier screening autosomal and X – linked recessive disorders

Abstract:

Screening přítomnosti patologických variant spojovaných s autozomálně recesivními a X-vázanými chorobami představuje efektivní strategii v reprodukčním zdraví a prevenci. V současné době se využívá několik různých přístupů pro tento druh screeningu, přičemž metoda masivně paralelního sekvenování má největší potenciální potenciál odhalit co nejvíce jedinců se zvýšeným rizikem narození potomka ovlivněného některou z vzácných chorob. Cílem této práce proto bylo zavést metodu masivně paralelního sekvenování v rámci prekoncepčního screeningu autozomálně recesivních a X-vázaných chorob na Oddělení lékařské genetiky Fakultní nemocnice Brno. Za tímto účelem bylo třeba navrhnout panel genů autozomálně recesivních a X vázaných chorob, který by co nejlépe odpovídal potřebám prekoncepčního screeningu. Vhodnost metody byla vyzkoušena vyšetřením souboru šesti pacientských vzorků a srovnáním výsledků masivně paralelního sekvenování s výsledky vyšetření podskupiny čtyř vzorků dalšími metodami používanými pro prekoncepční screening přenašečství. Výsledky masivně paralelního sekvenování se neodlišovaly od výsledků scoringových metod (AMRS, multiplexní PCR a reverzní hybridizace, real time PCR, analýza křivek tání) a toto srovnání umožnilo určit výhody a limity masivně paralelního sekvenování při aplikaci na prekoncepční screening přenašečství. Shoda výsledků naznačuje, že masivně paralelní sekvenování je vhodná metoda k detekci sekvenčních variant v genech asociovaných s autozomálně recesivních a X vázaných chorob v rámci prekoncepčního screeningu přenašečství a je použitelná v běžné klinické praxi.

Abstract:

Screening for pathological variants associated with autosomal recessive and X-linked diseases is an effective strategy in reproductive healthcare and prevention. Several different approaches are currently being used for this type of screening, with the massively parallel sequencing presenting the greatest potential to detect most individuals with an increased risk of having an offspring affected by a rare disease. The aim of this thesis was to introduce a method of massively parallel sequencing within the preconception screening of autosomal recessive and X-linked recessive diseases at the Department of Medical Genetics of the University Hospital Brno. To achieve this, a panel of autosomal recessive and X-linked disease genes was designed to meet the needs of preconception carrier screening. The suitability of the method was then tested by examining a set of six patient samples and comparing the results of massively parallel sequencing with the results of other methods used on a subset of four samples. The results of massively parallel sequencing did not differ from the results by scoring methods (AMRS, multiplex PCR and reverse hybridization, real time PCR), which revealed the benefits and limits of massively parallel sequencing applied to preconception carrier screening. These results show that massively parallel sequencing is a suitable method for detection of variants associated with autosomal recessive and X-linked recessive diseases as part of preconception carrier screening and that the method is applicable in routine clinical practice.
