Cirkulující endoteliální a progenitorové buňky jako biomarkery angiogeneze u dětských onkologických pacientů

Circulating endothelial and progenitor cells as biomarkers of angiogenesis in pediatric oncology patients

Abstract:

Novotvorba cév angiogenezí či vaskulogenezí je nedílnou součástí nádorového růstu. Má zásadní význam pro jeho invazivitu a metastazování. S rozvojem antiangiogenní léčby se objevila potřeba spolehlivého markeru angiogeneze, jenž by měl jak prognostický, tak prediktivní význam. Cirkulující endoteliální buňky (CEC) a cirkulující endoteliální progenitorové buňky (EPC) byly několika studiemi navrženy jako potenciální neinvazivní biomarkery nádorové angiogeneze. Cílem naší práce byla podrobná analýza množství CEC a EPC u dětských onkologických pacientů jak iniciálně ve vztahu k typu nádoru, rozsahu onemocnění, typu dosažené odpovědi na léčbu, tak i monitorace dynamiky těchto buněk v průběhu léčby, srovnání konvenční chemoterapie a metronomické/antiangiogenní léčby, zhodnocení vlivu febrilní neutropenie, operačního zákroku či podávání rekombinantního G-CSF. Analýza cílů v první etapě výzkumu byla provedena u 171 dětí. Do druhé části studie bylo zařazeno 30 pacientů. Pro zhodnocení vlivu febrilní neutropenie, operačního zákroku a podávání Neupogenu bylo analyzováno celkem 478 vzorků periferní krve. Množství buněk bylo stanoveno pomocí flowcytometrie. V první etapě byly hodnocené aktivované CEC (aCEC) s imunofenotypickou definicí CD45-CD146+CD105+, zralé CEC (mCEC) CD45-CD146+CD31+ a EPC CD45-/+CD146+CD34+. Ve druhé etapě byly hodnoceny CEC jako CD45-CD146+CD31+, EPC jako CD45-CD146+CD133+ a jejich mrtvé/apoptotické varianty 7-AAD+. V našem souboru jsme nenalezli signifikantní rozdíl v iniciálním množství sledovaných buněk v korelaci s typem nádoru. Při srovnání hodnot před léčbou a v jejím průběhu bez ohledu na diagnózu došlo k poklesu v množství aCEC a mCEC. Při korelaci s odpovědí na léčbu byl patrný statisticky signifikantní pokles aCEC u pacientů, kteří dosáhli pouze stabilizace onemocnění nebo u nichž došlo k progresi ve srovnání s pacienty, jež dosáhli objemové regrese tumoru. Rozsah onemocnění a léčby neměl významný vliv. Po podání konvenční chemoterapie došlo k signifikantnímu nárůstu CEC i EPC v průběhu prvních 7 dnů bloku a následně k poklesu hodnot, zatímco dynamika sledovaných buněk v průběhu antiangiogenní léčby nevykazovala žádný výraznější trend. Iniciálně v průběhu febrilní neutropenie a po podání Neupogenu došlo k signifikantnímu poklesu CEC i EPC, zatímco operační zákrok neměl významný vliv na sledované hodnoty. V naší studii jsme tedy nepotvrdili význam EPC a CEC jako spolehlivých biomarkerů u dětských onkologických pacientů. V praxi by bylo možné využít monitoraci aCEC k vytipování pacientů, kteří by mohli hůře odpovídat na léčbu. Při podávání konvenční chemoterapie může docházet k dočasné stimulaci neoangiogeneze, zatímco při antiangiogenní léčbě k výraznějším výkyvům v hodnotách EPC a CEC nedochází. Pro další využitelnost EPC a CEC je nutný standardizovaný a robustně validovaný protokol pro analýzu pomocí flowcytometru.

Abstract:

Neovascularization by angiogenesis or vasculogenesis is an integral part of the tumor growth, it is essential for tumor invassivness and for the process of metastasis. With the development of antiangiogenic therapies there is urgent need for reliable biomarkers of angiogenesis, that would have both - prognostic and predictive meaning. Circulating endothelial cells (CEC) and circulating endothelial progenitor cells (EPC) have been proposed as a potential noninvasive biomarker of tumor angiogenesis in several studies. The aim of our work was a detailed analysis of the amount of CEC and EPC in pediatric cancer patients, both initially in relation to tumor type, extent of the disease, type of the treatment response as well as monitoring the dynamics of these cells during treatment, comparison of conventional chemotherapy and metronomic/antiangiogenic treatment, an evaluation of the effect of febrile neutropenia, surgery, or administration of recombinant G-CSF. The analysis of objectives in the first part of the research was performed in 171 children. The second part of the study included 30 patients. A total of 478 peripheral blood samples were analyzed to assess the effect of febrile neutropenia, surgery, and Neupogen administration. Cells were determined by flowcytometry. In the first part we evaluated activated CEC (aCEC) with immunophenotypic definition CD45-CD146+CD105+, mature CEC (mCEC) CD45-CD146+CD31+ and EPC CD45-/+CD146+CD34+. In the second part, CECs were evaluated as CD45-CD146+CD31+, EPC as CD45-CD146+CD133+ and their dead/apoptotic variants as 7-AAD+. Results: We did not find a significant difference in the initial amount of the monitored cells in correlation with the tumor type. Comparing the values before and during treatment, regardless of the diagnosis, there was a decrease in the amount of aCEC and mCEC. In correlation with the treatment response, there was a statistically significant decrease in aCEC in patients who achieved only disease stabilization or progressed on treatment compared to patients who achieved regression in the tumor volume. The extent of the disease and treatment did not have a significant effect. Following conventional chemotherapy, there was a significant increase in both CEC and EPC over the first 7 days of the chemotherapy cycle, followed by a decrease in values, while the dynamics of the monitored cells during the antiangiogenic treatment showed no significant trend. Initially, during febrile neutropenia and after Neupogen administration, there was a significant decrease in both CEC and EPC, whereas the surgery did not have a significant impact on the monitored values. In our study, we did not confirm the significance of EPC and CEC as reliable markers in pediatric cancer patients. In practice, aCEC monitoring could be used to identify patients who may be less responsive to treatment. Conventional chemotherapy may temporarily stimulate neoangiogenesis, whereas anti-angiogenic therapy does not result in significant fluctuations in EPC and CEC values. For the further use of EPC and CEC as biomarkers of angiogenesis, a standardized protocol for flowcytometry analysis is required.
