Prognostic immunohistochemistry of soft tissue sarcomas

Abstract:

Na souboru 101 primárních sarkomů měkkých tkání končetin a trupu dospělých pacientů, neléčených jinak než chirurgicky byla nepřímou imunohistochemií vyšetřována exprese proteinů s potenciálním prognostickým významem, proliferačního antigenu Ki-67, p53, mdm-2, cyklinu D1, p16, nm-23 a EGFR, kromě Ki-67 s použitím tkáňových microarrays. Soubor zahrnoval sarkomy různých histologických typů a subtypů, a byl tvořen 15 sarkomy s histologickým grade 2, 28 tumory histologického grade 3 a 58 sarkomy grade 3. Frakce proliferujících buněk (Ki-67 proliferační index) byla v rozmezí 1,3 – 75,5 %. Nukleární akumulace p53 byla zaznamenána u 25 případů (24,75 %), overexprese mdm-2 u 63 případů (62,38 %). Overexprese cyklinu D1 byla pozorována u 29 případů (28,71 %). Exprese p16 byla alterována u 94 případů (93,07 %), a nm-23 u 42 případů (41,58 %). EGFR byl pozitivní u 45 (44,55 %) z celkového počtu 101 sarkomů měkkých tkání Byla prokázána statisticky vysoce signifikantní korelace Ki-67 proliferačního indexu s počtem mitotických figur a Ki-67 proliferačního indexu, exprese p53, cyklinu D1 a EGFR s histologickým grade sarkomů, stanoveným dle schématu FNCLCC. Výsledky studie ukazují na možnost využít imunohistochemicky vyšetřovanou expresi p53, cyklinu D1, EGFR a Ki-67 proliferační index jako užitečný doplněk konvenčních histologických gradingových metod u sarkomů měkkých tkání, zvláště u v současné době populárních malých bioptických vzorků, které jsou pro klasické gradingové systémy málo vhodné.

Abstract:

101 patients, 51 men and 50 women, from 1989 to 2008, were diagnosed with a primary adult soft tissue sarcoma of the trunk and extremities. To identify immunohistochemical markers of potential prognostic significance in adult soft tissue sarcomas and to correlate their expression with the histological grade, the immunohistochemical analysis of Ki-67, p53, mdm-2, cyclin D1, p16, nm-23 and EGFR was carried out on tissue microarrays constructed from the formalin-fixed paraffin-embedded tissue blocks, except Ki-67 labelling index, for which the whole tissue block sections were used. The cases consisted of various types of soft tissue sarcoma, 15 tumours were histological grade 1, 28 sarcomas were grade 2 and 58 tumours were grade 3 determined according to the FNCLCC scheme. The fraction of proliferating cells – Ki-67 labelling index – varied from 1.3 to 75.5 %. Nuclear accumulation of p53 was observed in 25 cases (24.75 %) and overexpression of mdm-2 in 63 tumours (62.38 %). Overexpression of cyclin D1 was seen in 29 cases (28.71 %). Expression of p16 and nm-23 was altered in 95 (94%) and 42 (41.58 %) tumours, respectively. EGFR positivity was found in 45 sarcomas (44.55 %). There was strong statistical correlation between the Ki-67 labelling index, the nuclear accumulation of p53, expression of cyklin D1 and EGFR and the histological grade. The results of this study show that the immunohistochemical evaluation of expression of p53, cyclin D1, EGFR and Ki-67 labelling index is potential valuable adjunct of the conventional grading systems for soft tissue sarcomas, especially in the currently popular small core needle biopsy samples.
