Optimization of diagnostics and therapy in patients with acute myeloid leukemia older than 60 years

Abstract:

Akutní myeloidní leukémie (AML) je maligní klonální onemocnění krvetvorby, jehož incidence narůstá s věkem. U starších pacientů je onemocnění charakterizováno častějším výskytem prognosticky nepříznivých faktorů, které vedou k nedostatečné terapeutické odpovědi, vysokému procentu primárně refrakterních AML a častým relapsům. Indukční léčba chemoterapií bývá krom toho doprovázena často významnou toxicitou a léčba paliativní je spojená s velmi nízkým přežíváním nemocných. Disertační práci tvoří obecný úvod a dále pak komentovaný soubor publikovaných prací týkajících se terapie starších pacientů s AML a poznatků směřujících k její optimalizaci. Provedli jsme analýzu real-world dat na dosud největším publikovaném souboru AML pacientů ≥ 60 let v České republice a prokázali jsme, že kurativní léčba významným způsobem prodlužuje přežívání těchto nemocných. Identifikovali jsme nový cytogenetický prognostický faktor, který je spojen s agresivním průběhem onemocnění a vysokou mírou relapsů. Potvrdili jsme negativní dopad FLT3-ITD mutace u CBF leukémií. Na základě podrobného studia nepříznivých prognostických faktorů jsme navrhli rozhodovací algoritmus, který lze využít v terapeutické rozvaze k identifikaci pacientů, kteří budou profitovat z intenzivní terapie. Prokázali jsme, že infúze haploidentických periferních krvetvorných buněk následující po standardní indukční a konsolidační chemoterapii nezlepšuje terapeutické výsledky ani přežívání starších nemocných. Potvrdili jsme schopnost xenotransplantačních myších modelů simulovat klonalitu AML. Podíleli jsme se na tvorbě doporučených postupů pro prevenci a léčbu extravazace chemoterapeutik u hematoonkologických pacientů. Zpracovaná témata a výsledky předkládané disertační práce přispěly k optimalizaci léčby starších nemocných a AML.

Abstract:

Acute myeloid leukemia (AML) is malignant clonal disease of hematopoiesis the incidence of which increases with age. In case of older patients, the disease is characterized by a more frequent occurrence of negative prognostic factors that lead to poor therapeutic outcomes, high rate of primary refractory AML and to frequent relapses. The induction chemotherapy is often connected with high toxicity and the palliative treatment with very low survival rate. This thesis is formulated as a general overview and a composition of published papers focusing on therapy of elderly AML patients and its further optimization. We performed an analysis of real-world data on so far, the biggest published database of AML patients  60 in the Czech Republic and we proved that the curative treatment significantly prolongs the overall survival. We identified a new cytogenetic negative prognostic factor associated with an aggressive progress of the disease and a high rate of relapse. We confirmed a negative impact of FLT3-ITD on CBF-AML. Based on a detailed review of the poor prognostic factors, we devised a decision-making algorithm that can be used in therapeutic balance in order to identify patients who will benefit from the curative treatment. We proved that infusion of haploidentical peripheral blood stem cells ensuing the induction and consolidation chemotherapies does not improve neither the therapeutic outcomes nor the overall survival of the elderly patients. We confirmed the ability of mice xenotransplantation models to simulate AML clonality. We participated on the creation of guidelines for prevention and treatment of chemotherapeutics extravasation in haemato-oncological patients. In conclusion, all the results mentioned in this thesis contributed significantly to the elderly AML treatment optimization.
