Development and validation of combined response inhibition and vigilance continuous performance task

Abstract:

Porucha pozornosti s hyperaktivitou v dospělém věku je značně heterogenní poruchou, jejíž diagnostika je velice obtížná. V zahraničí jsou jedním z nejfrekventovanějších testů, které mohou napomoci diagnostice neuropsychologických deficitů u ADHD, testy kontinuálního výkonu. Jedná se o značně heterogenní skupinu testů, které pravděpodobně měří mírně odlišné konstrukty. V České republice počítačem administrovaný test kontinuálního výkonu použitelný u dospělé populace zatím chybí. Tento nedostatek se snažíme v této práci překonat. Vytvořili jsme test kontinuálního výkonu, který by mohl pomoci s diagnostikou deficitů v oblasti reakční inhibice či udržování pozornosti a dále by tento nástroj mohl pomoci sledovat mírné výkonové výkyvy, které jsou pro ADHD typické. Námi vytvořený test vykazuje vysoké úrovně vnitřní konzistence jednotlivých skórů a koreluje se sebehodnocením na sebeposuzovací škále pro ADHD v dospělosti, stejně jako se sebehodnocením každodenních kognitivních omylů spojených s pozorností. Zdrojové kódy tohoto nástroje a užité stimulové sady budou volně dostupné pro výzkumnou a klinickou veřejnost. Věříme, že zpřístupnění nástroje širší výzkumné a klinické veřejnosti v České republice pomůže zvýšit kvalitu výzkumu a diagnostiky u ADHD v dospělosti.

Abstract:

The Attention Deficit Hyperactivity Disorder (ADHD) in adulthood is a highly heterogeneous disorder that might be very difficult to diagnose. Worldwide, the most frequently employed tools enhancing the assessment of neuropsychological deficits in ADHD are the Continuous Performance Tasks. This is a largely heterogeneous group of tasks, each of which presumably measures a somewhat different construct. In the Czech Republic, computerised CPTs have not yet been introduced in neuropsychological assessment of adults. The present thesis attempts to fill this gap by introducing and providing validation data for a newly designed CPT that might aid the assessment of potential deficits in reactive inhibition and sustained attention and the monitoring of mild performance fluctuations, which are typical of ADHD. The results show that the new CPT exhibits high internal consistency of the individual scores and correlates positively with self-reported ADHD symptoms as measured by an adult ADHD scale as well as with self-reported attention-related cognitive failures. The source codes for the new tool together with the stimulus sets used in the study will be made freely available for research and clinical purposes. I believe that the public accessibility of the tool may improve the quality of adult ADHD research and assessment in the Czech Republic.
