The optimal time for umbilical cord clamping after physiological delivery

Abstract:

Problematika načasování přerušení pupečníku není v současné době příliš diskutovaným tématem, debata o něm však pomalu začíná nabývat na síle. Pozdní přerušení pupečníku není na porodních sálech v České republice standardem, ačkoli tento postup může významně prospět jak průběhu třetí doby porodní, tak adaptaci novorozence i jeho pozdějšímu vývoji. Tím, že necháme pupečník tepat déle a nepodvážeme jej ihned po porodu dítěte, umožníme fetální krvi z placenty přesun pupečníkem k dítěti – tzv. fetoplacentární transfuzi, což napomůže lepší adaptaci novorozence. Dalším přínosem odloženého přerušení pupečníku je snazší odloučení a porod placenty a tím i možné zkrácení trvání třetí doby porodní. Nestandartním přáním některých žen je nepřerušit pupečník vůbec – takovýto alternativní způsob porodu nazýváme lotosový porod.

Abstract:

The issue of the timing of cord clamping is not a much-discussed topic at the moment, although it is already starting to surface. Delayed cord clamping is not standard in delivery rooms in the Czech Republic, although this procedure can significantly benefit the course of the third stage of labour, the adaptation of the newborn and its later development. It allows the remaining blood from the placenta, called fetoplacental transfusion, to move through the umbilical cord to the baby – this will help the newborn to adapt better. Another benefit is the easier separation and delivery of the placenta and thus a possible reduction in the duration of the third stage of labour. However, the umbilical cord does not have to be interrupted at all after birth – this alternative method of delivery is called the lotus birth.
