Zhodnocení účinnosti neoadjuvantní chemoterapie u pacientek s karcinomem prsu a identifikace prediktivních a prognostických faktorů

Evaluation of Neoadjuvant Chemotherapy Response in Breast Cancer Patients and Identification of Predictive and Prognostic Factors

Abstract:

Neoadjuvantní chemoterapie (NACHT) patří mezi základní léčebné metody karcinomu prsu bez vzdáleného metastatického postižení. Účinnost NACHT ovlivňuje nejenom rozsah následného operačního výkonu, ale i další prognózu pacientek. V rámci disertační práce byly na konsekutivním retrospektivním souboru 784 pacientek komplexně popsány klinické, histopatologické, molekulárně-genetické a laboratorní charakteristiky jednotlivých podtypů karcinomu prsu, srovnáno hodnocení léčebného efektu NACHT dle zobrazovacích metod s definitivním histopatologickým nálezem po operačním výkonu a zhodnocena účinnost NACHT pomocí patologické kompletní remise (pCR) a dlouhodobých léčebných parametrů (OS, overall survival a RFS, relapse-free survival). Následně byla ověřena úloha prediktivních faktorů léčebné odpovědi a prognostických faktorů dalšího vývoje onemocněním, které jsou standardně používány v klinické praxi. U pacientek s triple negativním karcinomem (TNBC, triple-negative breast cancer) byla provedena podrobná analýza účinnosti platinových derivátů v NAT pomocí zhodnocení časné klinické odpovědi (ECR, early clinical response), pCR a dlouhodobých parametrů léčebné odpovědi (RFS a OS). V kombinaci s dalšími prediktivními faktory byly tímto způsobem identifikovány pacientky s největším benefitem z léčby platinovým derivátem. Dosažení ECR a pCR bylo signifikantně asociováno s lepším RFS a OS. Přidání platinového derivátu u pacientek s TNBC vede také ke zlepšení těchto parametrů, avšak statistické významnosti dosaženo nebylo. Poznatky získané touto prací mohou být využity v klinické praxi.

Abstract:

Neoadjuvant chemotherapy (NACT) is the standard treatment option for non-metastatic breast cancer. The effectiveness of NACT affects not only the extent of subsequent surgery but also the patient´s further prognosis. In this thesis, the clinical, histopathological, molecular-genetics, and laboratory characteristics of breast cancer subtypes were completely described on the retrospective consecutive cohort of 784 breast cancer patients treated with NACT. The comparison between the evaluation of NACT effectiveness according to the imaging and definitive histopathological assessment was performed. The effectiveness of NAT was evaluated using pathological complete response (pCR) and long-term treatment outcomes (RFS, relapse-free survival; OS, overall survival). Subsequently, the role of treatment response predictors and prognostic factors standardly used in daily clinical practice were verified. Detailed analysis of the efficacy of platinum salts in NACT of triple-negative breast cancer (TNBC) was performed by evaluating the early clinical response (ECR), pCR, and long-term treatment outcomes (RFS and OS) and in combination with other predictive parameters, patients with the greatest benefit of platinum salts addition were identified. Achieving ECR and pCR was strongly associated with favorable RFS and OS parameters. The administration of platinum salts was also associated with favorable RFS and OS parameters, but without reaching statistical significance. The findings and results may have immediate use in clinical practice.
