Autoimmune diseases of the thyroid gland and its connection with pregnancy and oncology

Abstract:

Bakalářská práce se zaměřuje na význam vyšetření protilátek proti štítné žláze u těhotných a u osob s nádorovým onemocněním. Práce má dvě části - teoretickou a praktickou. V teoretické části je popsána anatomie, histologie a fyziologie štítné žlázy, charakteristika hormonů a protilátek proti štítné žláze. Dále jsou popsána autoimunní onemocnění štítné žlázy, při kterých dochází ke vzniku protilátek proti štítné žláze, jedná se především o Hashimotovu tyreoiditu a Gravesovu-Basedowovu chorobu. Závěr teoretické části je věnován současným možnostem laboratorní diagnostiky autoimunních onemocnění štítné žlázy, včetně popisu jednotlivých laboratorních metod. Těžištěm práce je praktická část, která se zabývá statistickým zpracováním a retrospektivním i prospektivním vyhodnocením vyšetření parametrů štítné žlázy - TSH, fT4 a anti-TPO u těhotných a nemocných s nádorovými onemocněními. Využit byl klinický materiál Oddělení imunochemické diagnostiky Fakultní nemocnice v Plzni mezi lety 2002 až 2020. Závěrem práce jsou konkrétní doporučení pro vyšetřování parametrů štítné žlázy u těhotných žen a onkologických pacientů v klinické praxi.

Abstract:

The bachelor thesis focuses on the importance of thyroid antibody testing in pregnant women and people with cancer. The thesis has two parts - theoretical and practical. The theoretical part describes the anatomy, histology and physiology of the thyroid gland, the characteristics of hormones and thyroid antibodies. In addition, autoimmune thyroid diseases in which thyroid antibodies are produced are described, in particular Hashimoto's thyroiditis and Graves-Basedow disease. The theoretical part concludes with the current possibilities of laboratory diagnostics of autoimmune thyroid diseases, including a description of individual laboratory methods. The focus of the thesis is the practical part, which deals with statistical processing and retrospective and prospective evaluation of thyroid parameters - TSH, fT4 and anti-TPO in pregnant women and patients with cancer. The clinical material of the Department of Immunochemical Diagnostics of the University Hospital in Pilsen between 2002 and 2020 was used. The conclusion of the thesis are specific recommendations for the investigation of thyroid parameters in pregnant women and cancer patients in clinical practice.
