Women's breast cancer prevention in the Czech republic

Abstract:

Karcinom prsu je nejčastějším zhoubným nádorem u žen ve vyspělých zemích a představuje v současnosti jeden z nejzávažnějších zdravotnických problémů i v rámci ČR. Incidence má celosvětově stoupající tendenci, snižuje se však mortalita, což lze vysvětlit zavedením programů skríningového vyšetření. Cílem práce je formou dotazníkového šetření zjistit míru informovanosti laické populace o prevenci této nemoci a to na primární, sekundární i terciální úrovni. Průzkumu se zúčastnilo 50 respondentek, z nichž 40 % tvořily ženy ve věku do 20 let a 60 % ve věku 21-30 let. Znalost problematiky rakoviny prsu u oslovených subjektů byla nedostačující. Otázky, tykající se primární a sekundární prevence, většina část respondentek nezodpověděla správně. Z dotazníkového průzkumu vyplynulo, že 40 % žen má onemocnění prsu v rodinné anamnéze, z toho v 34 % se jednalo o rakovinu prsu. I když na pravidelné gynekologické preventivní prohlídky chodí 98 % oslovených respondentek, u 70 % z nich gynekolog neprovádí vyšetření prsu a neinformuje pacientky o správném způsobu samovyšetřování. Pravděpodobně i proto až 56 % oslovených žen samovyšetření prsu neprovádí. Jenom necelá polovina (46 %) respondentek zná název specifického přístrojového vyšetření pro včasnou diagnostiku mamograf a 4 % možnost rtg. vyšetření. Informovanost o skríningových programech v rámci sekundární prevence rakoviny prsu ještě není na dostatečné úrovni. Ukazuje se, že respondenti získávají informace zejména z médií a to především díky kosmetické společnosti Avon, což není uspokojivý výsledek. Do propagace sekundární prevence rakoviny prsu jako významné příčiny morbidity a mortality u populace ČR by se rozhodně měla ve větší míře zapojit odborná zdravotnická veřejnost

Abstract:

Breast cancer is one of the most common cancers in women in developed countries hence one of the most discussable health issue in the Czech Republic. The incidence of breast cancer has been increasing, however the mortality has been decreasing in the last three decades, due to breast screening implementation. The aim of the thesis is to explore the breast cancer awareness as well as its primary, secondary and tertiary prevention of the public population in the Czech Republic. Fifty women participated in this project; 40% of them were under 20 years old and 60% aged 21-30 years old. The qualitative questionnaire was used and overall, level of knowledge of these women were not sufficient. Most of the participants did not answer correctly the questions related to primary and secondary care. Forty per cent of participated women reported a family history of breast cancer. Although 98% of women attend regular check ups with their gynaecologist, 70% of women reported that their gynaecologist does not do breast check ups and do not teach women how to self examine their breasts. It seems like this is the main reason why 56% of these women do not self examine their breasts. Only 46% of the respondents know a mammography for the early detection and 4% know the mammography as the X-ray. The level of providing the appropriate information regarding breast screening as a part of the secondary prevention is very low. It seems that the public population get the information from the media, especially from Avon cosmetic company. This is not very satisfactory evidence. Therefore the secondary prevention of breast cancer as one of the main cause of morbidity and mortality within the Czech population should be more connected and supported by the Public Health Sector
