Měření elektrických dějů u izolovaných buněk: Vliv ethanolu na elektrofyziologické vlastnosti srdeční buňky pracovního myokardu

Abstract:

Nadměrné chronické požívaní alkoholu způsobuje řadu závažných onemocnění kardiovaskulárního systému jako změny funkce levé srdeční komory, dilatující kardiomyopatii či dokonce náhlou srdeční smrt. Nárazová nadměrná konzumace může vést ke vzniku arytmií (tzv. „holiday heart syndrome“). Zjištěno bylo i prodloužení všech fází elektrokardiogramu (EKG) člověka po aplikaci ethanolu. Přes znalost těchto negativních dopadů dosud nebyla účinkům ethanolu na elektrofyziologické vlastnosti srdeční tkáně věnována dostatečná pozornost. V této práci byl zkoumán vliv ethanolu na izolované komorové srdeční buňky potkana metodou whole-cell patch clamp při pokojové teplotě. Bylo zjištěno statisticky signifikantní zpomalení fáze rychlé depolarizace (dV/dt)max a snížení amplitudy akčního napětí AAN po aplikaci 2% ethanolu. Dále bylo pozorováno zkrácení délky trvání akčního napětí APD. U všech hlavních iontových membránových proudů byla zjištěna reverzibilní koncentračně závislá blokáda. V případě sodíkového proudu INa a vápníkového proudu ICa-L působil ethanol pokles amplitudy s hodnotami koncentrace působící 50% blokádu IC50 = 2.59 a 3.83 %. U draslíkového proudu Ito bylo navíc pozorováno zrychlení časového průběhu jeho poklesu během depolarizačního impulsu. Koncentrační závislost blokády Ito byla proto vyhodnocována ze změny jeho časového integrálu s výslednou IC50 = 13.17 %. Pozorované zrychlení poklesu Ito ukazuje na možnou interakci ethanolu s kanály v otevřeném stavu. Hillův koeficient h byl výrazně vyšší u INa (h = 2.43) oproti ICa-L (h = 1.01) a Ito (h = 0.80), což bývá interpretováno jako nutnost navázání více molekul látky působící blokádu. Z výsledků této studie lze uzavřít, že ethanol ovlivňuje elektrofyziologické vlastnosti srdečních buněk. Změny konfigurace akčního napětí odpovídají změnám průběhu jednotlivých membránových proudů. Blokáda INa je zodpovědná za snížení AAN a (dV/dt)max, zatímco zkrácení APD je velmi pravděpodobně způsobeno blokádou ICa-L.

Abstract:

A number of serious cardiovascular system diseases such as alterations of the left ventricular function, dilated cardiomyopathy, or even sudden cardiac death have been related to heavy chronical alcohol drinking. Even a heavy episodic consumption may lead to arrhythmias (the so-called „holiday heart syndrome“). A prolongation of all electrocardiogram parameters has been observed in human after administration of ethanol. Unfortunately, data describing the effect of ethanol on the electrophysiological properties of cardiomyocytes is rare. The effect of ethanol on isolated rat ventricular myocytes was investigated by the whole-cell patch clamp technique at room temperature. A statistically significant deceleration of the upstroke velocity and a decrease of the action potential amplitude were observed in the presence of 2 % ethanol. An action potential shortening was present as well. Ethanol reversibly inhibited all the main ionic membrane currents in rat in a concentration-dependent manner. A decrease in amplitude was documented in the case of sodium current INa and calcium current ICa-L with the concentration causing 50%-inhibition IC50 = 2.59 and 3.83 %, respectively. In the case of potassium current Ito, an acceleration of its decay during a depolarizing step was described. Thus, the concentration dependence of the Ito-inhibition was evaluated from changes of its time integral with the resulting IC50 = 13.17 %. Considering the acceleration of Ito-decay, the inhibition of Ito seems to be caused by an interaction of ethanol with the open channel state. The Hill coefficient h was much higher in the case of INa (h = 2.43) compared to ICa-L (h = 1.01) and Ito (h = 0.80) which is usually interpreted as a binding of more molecules inducing the inhibition. We conclude that ethanol alters electrophysiological properties of cardiomyocytes. The above mentioned variations of the action potential configuration are in a good agreement with the observed alterations of the measured ionic membrane currents. The inhibition of INa is responsible for the decrease of the action potential amplitude and the deceleration of its upstroke velocity whereas the action potential shortening is very likely caused by the inhibition of ICa-L.
