Abstract:

Úvod: V souvislosti s pandemií koronavirového onemocnění (COVID-19) došlo v naší republice k nové situaci a vláda svými nařízeními přistoupila k regulacím, ve většině případů nemohla standardně probíhat ani fyzioterapie. Někteří fyzioterapeuti začali pro kontakt s pacienty též využívat online konzultace, tedy jeden z přístupů telemedicíny. Myslím si, že telemedicína je velmi aktuální téma a vlivem COVIDU-19 se ukázal její význam a nutnost pracovat na jejím vývoji. Touto prací bych chtěla zjistit, jak by bylo možné potenciál telemedicíny aplikovat ve fyzioterapii. Cíl práce: Cílem teoretické práce je charakterizovat telemedicínu ve spojitosti s fyzioterapií, zhodnotit doporučené postupy telemedicíny, výhody a nevýhody telemedicíny ve fyzioterapii, analyzovat současnou situaci telemedicíny v ČR v rámci úhradových mechanismů a legislativního rámce. Cílem praktické části je zjistit faktory, které ovlivňují zájem klientů fyzioterapie o služby telemedicíny. Cílem praktické ukázky je předvést, jak by mohl být aplikovatelný distanční přístup ve fyzioterapeutické praxi. Metodika: Praktická část obsahuje rozbor dotazníků, který se zabývá faktory, které ovlivňují zájem klientů fyzioterapie o služby telemedicíny. Součástí je i ověřování hypotéz souvisejících s danou problematikou. Praktická ukázka distančního přístupu fyzioterapie byla zpracována formou videonahrávky. Výsledky: Celkově byl mírný zájem o používání telefonu pro přijímání a odesílání zpráv v rámci telemedicíny, zde odpovědělo 52 % respondentů, 47 % respondentů uvedlo, že má mírný zájem používat e-mailovou korespondenci. 34 % klientů fyzioterapie má mírný zájem využívat videohovory. Videohovory v rámci telemedicíny chce dle dotazníkového šetření využívat nejvíce klientů ve věku 18-29 let, odpovědělo zde 57 % respondentů. Nejmenší zájem využívat videohovory v rámci telemedicíny má věková kategorie 60-75+let, pouze 19 % klientů fyzioterapie. Zájem využívat videohovory v rámci telemedicíny se liší dle znalosti pojmu "telemedicína", věku respondentů, sebevědomí v dovednostech respondenta v této činnosti a dostupnosti počítače s videokamerou. Nepotvrdil se vztah mezi znalostí pojmu "telemedicína" a zájmem využívat telefonní konzultace v rámci telemedicíny. Závěr: Zavedení distanční přístupu do běžné praxe je vzdálenější než zavedení telemedicíny v lékařství. Distanční způsob fyzioterapie a rehabilitace v ČR funguje jen v rámci dílčích projektů a díky pandemii je dočasně umožněno provádět některé výkony fyzioterapie distanční formou. Zájem klientů o telemedicínu není veliký. Nejvyšší zájem je o využívání mobilních technologií. Během praktické ukázky distančního přístupu ve fyzioterapeutické praxi jsem pocítila náročnost organizační přípravy a potřebu technických a komunikačních schopností. Na druhou stranu vidím hodně výhod, které může přinést pacientům. Motivace, kontrola, získání informací, možnost využít tyto služby kdykoliv a kdekoli a to i v době infekčního rizika jsou hlavní klady, které distanční přístup přináší.

Abstract:

Introduction: In coherence with coronavirus pandemic (COVID-19) a new situation has arisen, and the government came up with regulations. In most cases, physiotherapy could not be conducted standardly. Some physiotherapists have moved to an online environment with their patients. This is one of many ways of telemedicine. I think it is a very actual topic due to COVID-19 that showed us the magnitude and the importance of telemedicine and forced us to improve it. In this thesis I would like to find out if it is possible to apply telemedicine in physiotherapy. Objective: The goal of the theoretical part is to characterise telemedicine in connection with physiotherapy, to evaluate recommended methods, advantages and disadvantages, to analyze the present situation of telemedicine in Czech Republic, in terms of reimbursement mechanism and legislative framework. The goal of the practical part is to showcase how a distant approach could be applied for a physiotherapeutic practice. Methodology: The practical part contains an analysis of questionnaires which are concerned with factors affecting client's interest in services of telemedicine. This part also contains validations of hypotheses related to this problematic. The practical showcase of a distant physiotherapy was videorecorded. Results: There was generally a slight interest in using a phone for receiving and sending messages within the framework of telemedicine. 52% respondents answered, 47% stated that they have a slight interest in using an e-mail correspondence, 34% clients of physiotherapy have a slight interest in using videocalls. In terms of telemedicine, videocalls are most wanted by the age group of people between 18-29 years, stated by 57% respondents. People in the age from 60 to 75+ years have the least interest in videocalls. That is only 19% clients of physiotherapy. Interest in using videocalls differs based on knowledge of the notion "telemedicine", age of respondents, confidence in skills in this procedure and availability of a computer with a camera. A relation between the knowledge of notion "telemedicine" and the interest in using phone consultations, in terms of telemedicine, has not been confirmed. Conclusion: Implementing a distant approach in usual practices is much further from functioning than an implementation of telemedicine in medicine. Distant method of physiotherapy and rehabilitation in Czech Republic works only within the framework of partial projects. It is temporarily possible to conduct some practices of physiotherapy via distant method thanks to the pandemic. Client's interest in telemedicine is not great. Mobile technologies have the utmost interest. During the practical showcase of the distant method in physiotherapy, I realised the difficulty of organizing a preparation, and the need of technical and communication skills. On the other hand, I can see it'll give many advantages to patients. Motivation, control, gain of information, option to use these services wherever and whenever, even in times of chance getting an infection, are the main pros that this distant method brings.
