Agressivity in health care institutions - perception of aggression by nurses

Abstract:

Cíl: Cílem průzkumu bylo zjistit výskyt agresivního chování ze strany pacientů vůči sestrám při výkonu povolání ve vybraném zdravotnickém zařízení. Dílčími cíli bylo zjistit pracoviště s nejvyšším výskytem agresivních pacientů, zjistit postoje sester k agresi pacientů. Metodika: Zkoumaný soubor tvořilo 222 sester pracujících ve sledovaném zdravotnickém zařízení. Dotazníkové šetření proběhlo od 1. 2. do 21. 2. 2013. Pro sběr dat byly zvoleny dva standardizované nástroje Výskyt agresie pacientov voči sestrám (VAPS) autorky Lepiešové a kol. (2009) a Škály postoje k agresi vůči sestrám ze strany pacientů (Attitudes Towards Aggression Towards ATAS?) autora G. J. Jansena (2005). Výsledky: Sestry hodnotily, jak často se v období posledního roku své ošetřovatelské praxe staly objektem agresivního chování ze strany pacienta. Nejčastěji se sestry setkávaly s verbálním útokem. Méně častým výskytem agresivního chování byl fyzický kontakt. Nejmenší výskyt agresivního chování byl tělesný kontakt se sexuálním záměrem s fyzickým ublížením. Nejvyšší výskyt agresivních pacientů měla neurologická JIP, omezovací prostředky byly nejčastěji používány na chirurgické JIP. Dále sestry vyjadřovaly svůj postoj k agresi pacientů na základě svých osobních zkušeností. Sestry téměř jednoznačně nesouhlasily s tím, že agrese může přinést pozitivnější vztah mezi sestrou a pacientem, naopak agresi pokládaly za nepříjemné a odpuzující chování. Jen minimum z nich absolvovalo školení v problematice prevence a řešení agresivity pacientů vůči sestrám. Závěr: Četnější výzkumy věnované problematice agresivního chování ve zdravotnictví a publikování výsledků pro potřebu pracovníků ve zdravotnictví by mohly být důležitým krokem v prevenci proti agresivnímu chování ve zdravotnictví.

Abstract:

Aim: The aim of the survey was to determine the incidence of aggressive behavior of patients towards nurses during their performance in selected in healthcare facilities. One of the milestones was to find such facility where the occurrence of violent patients was the highest, in order to find out about the feelings and experiences of nurses with patients aggression. Methods: The examinated group consisted of 222 nurses working in the monitored medical center. A questionnaire survey took place since Feb. 1st. till Feb. 21st. of 2013. For data collection were selected two standardized instruments Violence and Aggression of Patients Scale ? VAPS, author Lepiešová and co. (2009) and Scales of attitudes towards aggression against nurses by patients (Attitudes Towards Aggression Scale ? ATAS) Author G. J. Jansen (2005). Results: Nurses were evaluating how often during the last year of their nursing practice they have become the subject of aggressive behavior on the part of the patient. Most nurses met with verbal attacks. Less frequent occurrence of aggressive behavior was physical contact. The lowest incidence of aggressive behavior was physical contact with sexual intent and physical harm. The highest incidence of violent patients had neurological ICU (Intensive Care Unit), restraining measures were used most frequently in surgical ICU. Furthermore nurses were expressing their attitude to aggression patients based on their personal experience. Nurses almost unanimously disagreed with the fact that aggression can bring positive relations between nurse and patient, let alone they felt the aggression as unpleasant and repulsive behavior. Just a fraction of them underwent training on the prevention and resolution of patient aggression against nurses. Conclusion: More widespread and detailed studies dealing with the issue of aggressive behavior in healthcare and publishing of the outcomes for the workers in healthcare could be an important step in the prevention of aggressive behavior.
