Effects of a physiotherapy intervention on postural stability in Parkinson's disease

Abstract:

Parkinsonova nemoc je progresivní neurodegenerativní onemocnění, které kromě typických motorických příznaků ovlivňuje posturální stabilitu pacientů, čímž zvyšuje riziko pádů a omezuje kvalitu života. Cílem práce bylo posoudit efektivitu fyzioterapeutické intervence zaměřené na zlepšení posturální stability pacientů s Parkinsonovou nemocí, u nichž je vyšší riziko pádu v důsledku neurologického deficitu. Praktická část byla realizována formou kazuistické analýzy dvou pacientů diagnostikovaných Parkinsonovou nemocí. Posturální stabilita byla hodnocena pomocí standardizovaných testů (Balanční škála podle Bergové, Zkouška postavení a chůze na čas, Push and Release Test, Zkouška pěti postavení ze sedu) před a po osmitýdenním intervenčním programu, který zahrnoval balanční trénink, aktivaci hlubokého stabilizačního systému a nácvik motoricko-kognitivní koordinace. Celkové výsledky naznačují, že cílená fyzioterapie může pozitivně ovlivnit rovnováhu, funkční mobilitu a snížit riziko pádů u pacientů s Parkinsonovou nemocí. Závěrem lze konstatovat, že včasné zařazení fyzioterapeutické intervence je přínosné pro udržení soběstačnosti pacientů s Parkinsonovou nemocí.

Abstract:

Parkinson's disease is a progressive neurodegenerative disorder that, in addition to the typical motor symptoms, affects patients' postural stability, thereby increasing the risk of falls and limiting quality of life. The aim of this thesis was to assess the effectiveness of a physiotherapeutic intervention focused on improving postural stability in patients with Parkinson's disease who are at higher risk of falling due to neurological deficits. The practical part was carried out the form of a case study analysis of two patients diagnosed with Parkinson's disease. Postural stability was evaluated using standardized tests (Berg Balance Scale, Timed up and Go Test, Push and Release Test, Five Times Sit to Stand Test) before and after an eight-week intervention program that included balance training, activation of the deep stabilization system, and motor-cognitive coordination training. Overall results suggest that targeted physiotherapy can positively affect balance, functional mobility and reduce the risk of falls in patients with Parkinson's disease. In conclusion, early implementation of physiotherapeutic intervention is beneficial in maintaining self-sufficiency in patients with Parkinson's disease.
