Kinezioterapie po Boyd-McLeod a Mini-open operaci u pacientů s laterální epikondylitidou humeru

Kinesiotherapy after The Boyd-McLeod procedure and Mini-open muscle resection procedure with patients suffering from the lateral epicondylitis of humerus

Abstract:

Pacienti s laterální epikondylitidou humeru jsou častými návštěvníky rehabilitačních ambulancí, nicméně pohledy na léčbu se mezi odborníky liší. V případě selhání konzervativní terapie je u 7 % indikováno operativní řešení. Cílem práce je zhodnotit vliv kinezioterapie u sledovaných skupin pacientů na obnovení funkce horní končetiny v každodenním životě prostřednictvím standardizovaného dotazníku PRTEE (Patient-Rated Tennis Elbow Evaluation). Výzkumný soubor se skládal ze 14 probandů obou pohlaví (8 žen, 6 mužů, věk 48,5 ? 7,5 let; výška 174,5 ? 14,5 cm; hmotnost 83,7 ? 20 kg) s diagnózou chronické laterální epikondylitidy loketního kloubu. Odběr anamnestických dat a kineziologického vyšetření bylo provedeno v prostorách Lékařské fakulty Ostravské Univerzity v Ostravě nebo v Rehabilitačním zařízení MUDr. Gavlíček Hlučíně nebo v Ostravě. Pro zhodnocení funkčního deficitu a bolesti loketního kloubu v běžných denních aktivitách byl využit standardizovaný dotazník PRTEE a dynamometr Jamar pro posouzení síly stisku. Specifická pooperační kinezioterapie byla stanovena na 10 terapií o délce 60 min ve frekvencí dvakrát týdně. Byly zaznamenávány hodnoty před operací, po sundání sádrové fixace a po kinezioterapii. Výsledky ukazují, že operační metoda Mini-Open Muscle Resection je pro pacienty oproti přístupu Boyd-McLeod šetrnější a nabízí rychlejší návrat do běžných činností. U operace Mini-Open došlo ke statisticky významnému snížení bolesti a zlepšení funkce horní končetiny dle dotazníku PRTEE ve srovnání s Boyd-McLeod. Stejného výsledku bylo dosaženo u hodnocení síly stisku, kdy probandi po Mini-Open dosáhli statisticky významného zlepšení jak ve flektovaném, tak extendovaném lokti. Velkým benefitem tohoto přístupu je minimální zhoršení bolestivosti ihned po operaci. V problematice léčby laterální epikondylitidy humeru se jedná o unikátní práci vzhledem ke stále omezeně využívanému chirurgickému přístupu Mini-Open Muscle Resection v České republice. Nastiňuje možnosti efektivního a šetrného řešení této diagnózy s minimálním dopadem na běžný život pacienta.

Abstract:

Patients with lateral epicondylitis of the humerus are frequent visitors to rehabilitation clinics; however, perspectives on treatment vary between specialists. In case of failure of conservative therapy, surgical treatment is indicated in 7 %. The aim of this study is to evaluate the effect of kinesiotherapy in the studied groups of patients on the recovery of upper limb function in daily life by using the standardized questionnaire PRTEE (Patient-Rated Tennis Elbow Evaluation) questionnaire. The study population consisted of 14 probands of both sexes (8 females, 6 males, age 48,5 ? 7,5 years; height 174,5 ? 14,5 cm; weight 83,7 ? 20 kg) diagnosed with chronic lateral epicondylitis of the elbow joint. Collection of anamnestic data and kinesiological examination was performed at the premises of the Faculty of Medicine of the University of Ostrava in Ostrava or at the Rehabilitation Facility of MUDr. Gavlicek in Hlučín or Ostrava. A standardized PRTEE questionnaire and a Jamar dynamometer were used to assess the functional deficit and elbow joint pain in normal daily activities. Specific postoperative kinesiotherapy was set at 10 sessions of 60 minutes duration at a frequancy of twice a week. The Values were recorded before surgery, after removal of plaster fixation and after kinesiotherapy. The results show that the Mini-open Muscle Resection surgical method is gentler for patients compared to the Boyd-McLeod approach and offers a faster return to normal activities. There was a statistically significant reduction in pain and improvement in upper extremity function as assessed by the PRTEE questionnaire with the Mini-Open compared to the Boyd-McLeod. The same result was achieved for grip strength assessment, with probands achieving statistically significant improvement in both flexed and extended elbow after Mini-open. The major benefit of this approach is the minimal worsening of pain immediately after surgery. In the field of treatment of lateral epicondylitis of the humerus, this is a unique work due to the still limited use of the Mini-Open Muscle Resection surgical approach in the Czech Republic. It outlines the possibilities of effective and sparing treatment of this diagnosis with minimal impact on the patient's everyday life.
