Biomarkers of Brain Injury in Children - Potential Uses and Limitations

Abstract:

Traumatické poranění mozku (TBI) je nejčastější příčinou smrti u dětí. Určení závažnosti traumatického poranění mozku je obtížné. Dnes je většina diagnostických procesů založena na moderních zobrazovacích metodách jako jsou počítačová tomografie (CT) a magnetická rezonance (MRI). Počítačová tomografie má relativně malou citlivost pro zobrazení difuzního mozkového poškození a dostupnost magnetické rezonance je limitována. Z těchto důvodů je tedy u akutního poranění mozku potřeba doplňkových diagnostických testů - biochemické markery poškození buněk centrálního nervového systému. Většina těchto proteinových biomarkerů jsou mediátory odpovědi na zranění nebo pozdější buněčné poškození či smrt buňky. Tyto markery by nám měly poskytnout informace o závažnosti poranění, kromě toho by měly mít schopnost detekovat sekundární poškození a vést nás k optimalizaci terapie. Nakonec by měly poskytnout informace o prognóze pacienta do budoucna. Cílem této práce je korelace biomarkerů mozkového poškození u dětí se závažností poranění mozku, mortalitou a predikcí výsledného outcome.

Abstract:

Traumatic brain injury (TBI) is a leading cause of death and disability worldwide. Clinical examination has limited ability to assess injury severity or predict outcome. The identification of pathology-specific biomarkers can assist in the diagnosis and estimation of prognosis, and can serve as surrogate markers for monitoring the effectiveness of a treatment. Most of these biomarkers are mediators of responses to injury and subsequent cell damage or cell death. For patients with severe TBI, a biomarker may be helpful to predict which patients are likely to experience secondary insults and help prognosticate. The aims of this study were the correlations amongs biomarkers of brain injury in children with severity of brain injury, mortality and prediction of the final outcome.
