Changing of sperm parameters in relation to time interval since operation of varicocele

Abstract:

Greeks v prvním století pozoroval dilataci testikulárních vén s jejich regurgitace nad varleti a následně zjistil ipsilaterální testikulární atrofii. Mikrochirurgická operace varikokély představuje nejčastěji chirurgickou ovlivnitelnou operaci léčby mužské infertility. Autor sleduje měny spermiogramu po mikrochirurgické operaci varikokély. Uvedená práce komplexně předkládá problematiku varikokély, jejího výskytu a různých způsobů ošetření. Srovnává výsledky, kterých jsme dosáhli u souboru 217 pacientů s varikokélou s jinými výsledky z různých podobných studií z mnoha oblastí světa. Souhrn poznatků z těchto studií by měl sloužit jako perspektivní doporučení pro klinickou praxi, jak zvolit nejvýhodnější způsoby řešení varikokély s důrazem na minimální výskyt pooperačních komplikací, menší dyskomfort a krátkou pooperační rekonvalescenci. Studie prokázala, že mikrochirurgická varikokelektomie představuje bezpečnou, vysoce účinnou a ekonomicky nejvýhodnější ze všech dosud běžně užívaných metod. řešení varikokély. Díky této operační modalitě došlo k podstatné redukci pooperační hydrokély, recidiv varikokély a minimalizaci testikulární atrofie.

Abstract:

Greeks in first century have observed dilatation testicular vein with their regurgitation over testicles and subsequently found out ipsilateral testicular atrophy. Varicoceles are the most frequently encountered, surgically correctable cause male infertility. The author followed the changes of sperm parameters after microsurgical varicocelectomy. The work gives a complete summary on the topic of varicocele, it’s occurrence and different ways of treatment. Our results got from the set of 217 our patients with varicocele are compared to the results from other similar studies all over the world. The results of these studies should serve as a perspective recommendation for clinical practice, for selection of optimal way of treatment of varicocele, stressing the reducing of post operative complications, lowering discomfort and shortening surgical reconvalescence. The study has proved that the microsurgical varicocelectomy represents a safe, very efficient and economically the best method from all the so far used methods in varicocele treatment. Thanks to this method the rate of the postoperative hydrocele, varicocele recidives and testicular atrophy was reduced.
