Physiotherapy for patients with spinal cord injury fokusing on the specifics of respiratory physiotherapy

Abstract:

Úvod: U pacientů s vysokou míšní lézí jsou zásadním způsobem ovlivněny dechové funkce. Respirační fyzioterapie je u těchto pacientů, zejména v subakutní fázi, stěžejní součástí léčby. Cíl: Cílem práce je přispět k řešení problematiky dechových dysfunkcí pacientů po úrazu míchy. Metodika: Pomocí přístroje MicroRPM Pressure Meter vyšetřit maximální inspirační a expirační tlaky u pacienta s míšní lézí v období zahájení a po ukončení hospitalizace na Spinální jednotce - v subakutní fázi míšního traumatu. Součástí léčby bude intenzivní, individuálně zaměřená respirační terapie s využitím dechových trenažerů Treshold. Porovnáním výsledků vyšetření ve zpracované kazuistice bude vyhodnocen efekt terapie. Výsledky: Zlepšení úrovně maximálních inspiračních a expiračních ústních tlaků, pozitivní ovlivnění dechové mechaniky. Závěr: Vlivem terapie došlo k zvýšení síly dýchacích svalů, rozvíjení hrudníku a zlepšení stereotypu dýchání. Nedošlo k úplnému navýšení maximálního výdechového ústního tlaku do oblasti normálních hodnot. I tak mohl být pacient dekanylován a mohl tak úspěšně pokračovat v rehabilitaci dle dlouhodobého rehabilitačního plánu.

Abstract:

Introduction: Respiratory functions are significantly affected in patients with high level of spinal cord injury. Respiratory physiotherapy is a crucial part of treatment in these patients, especially in the subacute phase. Aim: The aim of the work was to help solve the respiratory problems in patients after spinal cord injury. Methodology: Examination of maximum inspiratory and expiratory pressures using the MicroRPM Pressure Meter in a patient with a spinal lesion at the onset and after hospitalization at the Spinal Cord Unit - in the subacute phase of spinal trauma. Part of the treatment will be intense, individually focused respiratory therapy using the breathing trainer Treshold. The effect of the therapy will be evaluated by comparing the results of the examination in the case report. Results: Improvement of maximum inspiratory and expiratory oral pressures, positive effect on breathing mechanics. Conclusion: The therapy has increased the strength of the respiratory muscles, developed the chest compliance and improved the breathing stereotype. The maximum expiratory oral pressure has not been increased to the range of normal values. Even so, the tracheostomy has been removed and pacient could have successfully continue the rehabilitation according the long term rehabilitation plan.
