Diagnostické markery v mozkomíšním moku u neurodegenerativních onemocnění

Abstract:

V této práci je zhodnocen význam měření hladiny betaamyloidu42 (Aβ42), celkového tau-proteinu a fosforylovaného tauproteinu v mozkomíšním moku pro diagnostiku vybrané skupiny neurologických onemocnění, zejména neurodegenerativních. Je posuzována senzitivita a specificita těchto biomarkerů pro účely diferenciální diagnostiky, závislost hladin na demografických proměnných, stupni onemocnění a tíži kognitivního deficitu. Do vyšetřovaného souboru byli zařazeni pacienti s Alzheimerovou chorobou (ACH), CreutzfeldtJakobovou demencí (CJD), frontotemporální demencí (FTD), klinicky izolovaným syndromem suspektním z roztroušené sklerózy (CIS), roztroušenou sklerózou mozkomíšní (RSM) a věkem odpovídající pacienti bez kognitivního postižení. Stanovení diagnóz se u pacientů opíralo o následující kritéria: Diagnostický a Statistický Manuál Americké Psychiatrické Asociace, 4. vydání (DSM IV), National Institute of Neurological and Communicative Disorders and Stroke and Alzheimer's Disease and Related Disorders Association (NINCDS – ADRDA) - kritéria pro diagnostiku ACH, Nearyho kritéria pro FTD, McDonaldova kritéria pro RSM a World Health Organization (WHO) kritéria pro pacienty s CJD. U pacientů byly pomocí ELISA metody stanoveny výše zmíněné markery v likvoru. Nemocní byli vyšetřeni pomocí zobrazovacích metod (MR či CT mozku) a bylo provedeno orientační vyšetření kognitivních a mnestických funkcí pomocí testu Mini Mental State Examination (MMSE). Naměřené zvýšené hodnoty koncentrací tauproteinu i fosfotauproteinu a snížení betaamyloidu42 v mozkomíšním moku u pacientů s ACH splňující námi požadovaný stupeň senzitivity/specificity nad 80 %. Tyto výsledky podporují vhodnost vyšetřování uvedených biomarkerů v diferenciální diagnostice ACH, s možností zahájení léčby i u počátečních a neúplně vyjádřených forem nemoci. Pro CJD se jako specifický marker jeví výrazná elevace hladin tau-proteinu. Využití tauproteinu a betaamyloidu42 v diagnostice roztroušené sklerózy se na základě našich výsledků nejeví jako přínosné. Odlišnost absolutních hodnot výsledků potvrzuje nutnost stanovení vlastních norem pro každou laboratoř

Abstract:

The aim of this work was to assess the importance of betaamyloid42, total tau-protein and phosphorylated tau-protein in the diagnostic process of selected neurological disorders, primarily neurodegenerative ones. Sensitivity and specificity of three selected biomarkers were measured for the purpose of differential diagnosis. Moreover, an association between the CSF level of given biomarkers and factors as demographical variables, stage of the disease and cognitive impairment was calculated. Patients with Alzheimer’s disease (AD), CreutzfeldtJakob disease (CJD), Frontotemporal dementia (FTD), Clinically isolated syndromes (CIS) and Multiple sclerosis (MS) were included into the study, as well as an age matched cognitive intact controls. Diagnoses were based on following criteria: DSM IV general criteria for dementia, National Institute of Neurological and Communicative Disorders and Stroke and Alzheimer's Disease and Related Disorders Association (NINCDS – ADRDA) criteria for Alzheimer’s disease, Neary’s criteria for FTD, McDonald‘s criteria for MS and World Health Organization (WHO) criteria for CJD. Described biomarkers were measured in all patients. Moreover, participants were examined by MRI or CT of the brain and were broadly assessed regarding their cognitive and memory status by means of Mini Mental Scale Examination (MMSE). Outcomes showing increased concentrations of total tau protein and phosphorylated tauprotein and decrease of betaamyloid42 levels in CSF of patients with AD reached required sensitivity/specificity ratio of 80 % or above. Presented results support an indication to examine three mentioned biomarkers as a part of differential diagnosis of AD, thus allowing the treatment to be initiated in early or not fully expressed forms of the disease. Based on study results a marked elevation of CSF levels of total tau protein is a specific finding in CJD. Based on our results, the possibility of using tau protein and beta-amyloid42 levels in the routine diagnosis of MS seems to be non-beneficial A diversity of results' absolute values proves a necessity to establish their own specific standards in each laboratory.
