Nontraumatic spinal extramedular haematomas

Abstract:

Spinální extramedullární hematom je poměrně vzácné onemocnění a jeho diagnostika byla donedávna dosti svízelná. Teprve magnetická rezonance nám dokázala přesně zobrazit míchu a páteřní kanál, zobrazit celý spinální hematom, s velkou pravděpodobností odlišit epidurální hematom od intradurálního. První popis MR obrazu spinálního hematomu je z roku 1990. Práce hodnotí soubor pacientů za jedenáctileté období při využití MR diagnostiky v našem ústavu. Sledovaný soubor zahrnuje jedenáct pacientů se spinálními hematomy, kteří byli operovaní v letech 1997 – 2007. Sedm hematomů se nacházelo subdurálně a čtyři epidurálně. 64% pacientů mělo antikoagulační terapii nebo jinou koagulační poruchu. Spinální hematom je akutní stav, který vyžaduje rychlou a adekvátní terapii. Je to diagnosa, na kterou musíme myslet v diferenciální diagnostice u pacientů s hematologickým onemocněním nebo s antikoagulační terapií, u kterých se náhle objevily příznaky komprese míchy nebo kořenů kaudy. Pacienti s podezřením na spinální hematom by měli být transferováni do specializovaných center, kde je dostupná vyšetřovací technika tj. magnetická rezonance se stálým provozem a kde mohou být akutně operovaní. Magnetická rezonance je nyní zlatým standardem pro vyšetření spinálního kanálu. Pacienti s neurologickým deficitem nebo progredujícím neurologickým deficitem, kteří jsou schopni operace musí být neodkladně operovaní. Výsledný efekt terapie závisí nejen na rozsahu a lokalizaci hematomu, závažnosti neurologického postižení – tj. na faktorech, které nejsme schopni ovlivnit, ale také na včasnosti chirurgické dekomprese. U pacientů, kde není velký neurologický deficit a operační výkon by byl značně rizikový, lze postupovat i konzervativně.

Abstract:

Spinal extramedular haematoma is a relatively rare disease and its diagnostics has until recently been quite difficult. Only magnetic resonance could exactly show the spinal cord and the spinal canal. It. could show well the whole spinal haematoma and could distinguish with great probability the epidural haematoma from the intradural haematoma. The first description of the MR picture of the spinal haematoma dates back to the year 1990. The work assesses the array of patients of our clinic in the period of eleven years with the use of MR diagnostics. It includes eleven patients with spinal haematomas operated in the years 1997 – 2007. Seven haematomas were subdural, four epidural. 64 % of patients had anticoagulation therapy or another coagulation disorder. Spinal haematoma is an acute state which requires quick and adequate therapy. It is a diagnosis which we must think of in differential diagnostics in patients with haematological disease or with anticoagulation therapy where suddenly symptoms of spinal cord or cauda root compression appeared. When there is a suspicion of spinal haematoma the patients should be transferred to specialized centres with examination technique, i.e. magnetis resonance available permanently and where the patients could be urgently operated. Magnetic resonance is now the golden standard in the examination of the spinal canal. The patients with neurological deficit or with progressive neurological deficit who are capable of being operated have to be operated without delay. The final effect of the therapy depends not only on the size and localization of the haematoma, the seriousness of the neurological affliction (the factors that we are unable to influence), but also on the timely surgical decompression. In the patients without a great neurological deficit and where the operation would be verry risky it is possible to proceed in a conservative way as well.
