Quality of healthcare in surgical clinics

Abstract:

Úvod. Kvalita zdravotních služeb a její neustálé zlepšování se dostaly do popředí zájmu ve zdravotnických zařízeních po celém světě, nevyjímaje chirurgickou péči. S narůstající náročností pacientů a celkovým posunem zaměření zdravotní péče na pacienta není možné podceňovat důležitost sledování jejich pohledů, očekávání a zkušeností. Zaměřili jsme se tedy na identifikování možných nedostatků v poskytované chirurgické péči, které ovlivňují spokojenost pacientů a jimi vnímanou kvalitu obdržených služeb. Metody. Výzkum byl proveden ve dvou etapách. V průběhu první fáze bylo za pomoci kvantitativních i doplňkových kvalitativních metod prošetřeno celkem 1 982 pacientů hospitalizovaných na třech českých a jednom britském chirurgickém oddělení / klinice. Druhá fáze zahrnovala vzorek celkem 934 českých ambulantních chirurgických pacientů. Nasbíraná data byla analyzována a srovnána za použití Chí-kvadrát testu. Výsledky. Srovnání českých a anglických hospitalizovaných pacientů (vnímané nedostatky versus vnímaná absence nedostatků) první fáze ukázalo, že kromě nemocniční stravy a narušení spánku existují statisticky významné rozdíly (p<0.05) u všech ostatních 12 sledovaných faktorů. Angličtí hospitalizovaní pacienti vnímali statisticky významně méně nedostatků než čeští pacienti. Během druhé fáze byly statisticky významné rozdíly (p<0.05) ve vnímaných nedostatcích identifikovány mezi českými hospitalizovanými a ambulantními pacienty ve všech sledovaných faktorech kromě pohledu na osobní prezentaci a hygienu zdravotního personálu. Zkušenost ambulantních pacientů, vyjádřena jako vnímané nedostatky, byla horší ve srovnání s hospitalizovanými pacienty, zejména v souvislosti s komunikací a vztahem s ošetřujícím lékařem. Závěr. Většina identifikovaných nedostatků v chirurgické zdravotní péči je preventabilní, protože nedostatky se primárně vztahují ke komunikaci zdravotního personálu s pacienty, ale také k tak základním hygienickým aspektům jako je mytí rukou. Je důležité mít na paměti, že pacienti vnímají i takové detail. Žijeme ve společnosti, kde nároky i žaloby pacientů rostou, i proto by tyto nedostatky měly být odstraněny co nejdříve.

Abstract:

Background. The quality of health related services and its continuous improvement have become fundamental concerns across health care facilities worldwide, including the surgical care. With the increasingly demanding patients and the overall shift towards patient-centred care, the importance of monitoring patients’ perceptions, expectations and experiences cannot be underestimated. We focused on identifying possible deficiencies in provided surgical care that influence patient satisfaction and perceived quality of services received. Methods. The research was conducted in two stages. During the first stage, the total sample of 1,982 in-patients from three Czech a one U.K. surgical departments / clinics were surveyed using both quantitative and qualitative complementary methods. The second stage included a sample of 934 Czech outpatients. Collected data were analysed and compared by means of Chi-square test. Results. The comparisons between Czech and English in-patients (perceived deficiencies versus perceived lack of deficiencies) during the first stage showed that, except for hospital food and sleep discomfort, there were statistically significant differences (p<0.05) for all other 12 followed factors. The English in-patients perceived significantly less deficiencies than their Czech counterparts. During the second stage, statistically significant differences (p<0.05) in perceived deficiencies were identified between Czech in-patients and outpatients for all monitored factors but the opinion about staff personal presentation and hygiene. Outpatients’ experience, expressed as perceived deficiencies, was worse compared to hospitalized patients, particularly in relation to communication issues and relationship with the attending physician. Conclusions. The identified deficiencies in the surgical health care are mostly preventable since they are related primarily to health staff’s communication with patients, but also to such essential hygienic aspects as hand-washing. It is important to bear in mind that patients perceive these details. We live in a society where patients’ demands as well as litigations are increasing, so these deficiencies should be corrected as soon as possible.
