Frequency development of lethal injuries in traffic accidents in the catchment area of the Institute of Forensic Medicine, University Hospital Ostrava

Abstract:

Úvod: Dopravní úrazy jsou definovány jako všechna traumata, která mají souvislost s dopravními prostředky v pohybu. V posledních desetiletích bylo možno vnímat dlouhodobé kvantitativní změny zastoupení fatálních dopravních úrazů v kategorii násilných úmrtí. Cíl: Primárním cílem bylo zadokumentování všech obětí dopravních nehod ve spádové oblasti působení Ústavu soudního lékařství Fakultní nemocnice Ostrava v časovém intervalu let 2000-2020. Dále detailně diverzifikovat oběti dle střetů konkrétních typů dopravních prostředků. Dalším cílem bylo definování skupin faktorů a jejich významných změn podílejících se na frekvenci výskytu dopravních úrazů a jejich závažnosti. Metodika: Byl aplikován kombinovaný empirický epidemiologický retrospektivní výzkum. Důkazy získané sběrem dat vědeckými metodami případové studie zahrnovaly procházení a analýzu relevantních případů a reálných zkušeností týkajících se letálních poranění při dopravních nehodách na ÚSL FNO. Dále pro potřeby práce byly využity obě formy informační analýzy, tj. identifikační analýzy (knihy zemřelých) a obsahové analýzy (pitevní protokoly). Současně byla v prefázi použita identifikační analýza u statistik PČR. U kvalitativního testování byla pro popis znaků v souboru použita základní popisná statistika (program MS Excel, online program OpenEpi 3.01). Pro analýzu dat byly dále použity testy chí-kvadrát test a parametrický Studentův t-test (normalita dat byla testována pomocí Shapiro-Wilk testu). Byla zvolena 5% hladina významnosti. Výsledky: Analýzy v rámci spádové oblasti ÚSL FNO potvrdily dlouhodobý trend snižování letálních úrazů při dopravních nehodách. V roce 2002 se počty úmrtí dostaly na své maximum ve sledovaném období 2001-2020, minima dosáhly počty úmrtí v roce 2020. U dvoustopých vozidel (skupina I.) maximální hodnoty dosáhl trend úmrtnosti v roce 2004. Nejčastější příčinou úmrtí v rámci sledovaného období 2011-2020 byla diagnóza zhmoždění a rozhmoždění kmene mozkového následovaná roztržením a rozhmožděním srdečnice, triádu zakončuje úrazově-krvácivý šok. Závěr: V rámci zkoumaného období pro spádovou oblast ÚSL FNO je možné díky dlouhodobému trendu snižování počtu letálních úrazů při dopravních nehodách usuzovat, že jedním z podílejících se faktorů jsou zaváděná technická řešení ovlivňující bezpečnost dopravy. Pro přesnější dokazování by bylo nutné propojit soudně-lékařský výzkum s dopravně technickým. Dále výsledky šetření ukazují ve spádové oblasti ÚSL FNO na zásadní vliv zavedení bodového systému v roce 2005. V rámci dalších rozšíření výzkumu by bylo vhodné zaměřit se pro období 2011-2020 na druhou skupinu zahrnující další typy smrtelných dopravních nehod = cyklisté, motocyklisté, cestující drážními vozidly, nezařazená úmrtí a chodci.

Abstract:

Introduction: Road traffic injuries are defined as all traumas that are related to vehicles in motion. In recent decades, long-term quantitative changes in the representation of fatal road traffic injuries in the category of violent deaths have been perceived. Aim: The primary objective was to document all victims of road accidents in the catchment area of the Institute of Forensic Medicine of the University Hospital Ostrava in the 2000-2020 time period. Furthermore, to diversify victims in detail according to the collisions of specific types of vehicles. Another objective was to define groups of factors and their significant changes contributing to the frequency of road traffic injuries and their severity. Methodology: A combined empirical epidemiological retrospective research was applied. Evidence gathered through data collection using scientific case study methods involved reviewing and analyzing relevant cases and real-life experiences relating to fatal injuries in road traffic accidents at the Institute of Forensic Medicine of the FNO. Furthermore, both forms of information analysis, i.e., identification analysis (death registers) and content analysis (autopsy reports) were used for this dissertation. Simultaneously, the identification analysis was applied in the prephase with the statistics of the Police of the Czech Republic. For the qualitative testing, basic descriptive statistics (MS Excel, OpenEpi 3.01 online program) were used to describe the attributes in the file. Chi-squared test and parametric Student's t-test were also used for data analysis (normality of data was tested using the Shapiro-Wilk test). A significance level of 5% was chosen. Results: Analyses within the catchment area of the Institute of Forensic Medicine of the FNO have confirmed the long-term trend of decreasing fatal injuries in road traffic accidents. In 2002, the number of deaths reached its maximum in the 2001-2020 time period, and the number of deaths reached its minimum in 2020. For two-track vehicles (Group I), the mortality trend peaked in 2004. The most common cause of death in the 2011-2020 time period was a diagnosis of brain stem contusion and laceration followed by contusions and lacerations of the aorta, with traumatic hemorrhagic shock completing the triad. Conclusion: Due to the long-term trend of decreasing the number of fatal injuries in road traffic accidents within the study period for the catchment area of the Institute of Forensic Medicine of the FNO, it is possible to conclude that one of the contributing factors is the introduction of technical solutions affecting traffic safety. For more accurate evidence, combining forensic medical research with traffic engineering research would be necessary. Furthermore, the results of the investigation in the catchment area of the Institute of Forensic Medicine of the FNO show a major impact of the demerit points system implemented in 2005. Further extensions of the 2011-2020 time period research should focus on a second group including other types of fatal road traffic accidents = cyclists, motorcyclists, rail passengers, unclassified deaths, and pedestrians.
