Abstract:

Úvod: Onemocnění kardiovaskulárního systému (CVD) jsou celosvětově nejčastější příčinou úmrtí. Přičemž v některých případech je CVD komplikací jiného primárního onemocnění; CVD jako komplikace astmatu se vyskytuje v 34,3 % případů a v 65 % případů nádorových onemocnění. Proto je zásadní provádět vyšetření kardiovaskulárního systému (CV) také u nekardiovaskulárních chorob. Cílem této práce je sběr a vyhodnocení dat u astmatických dětí a pacientů s akutní lymfoblastickou leukémií (ALL) se zaměřením na stanovení změn v CV. U pacientů s ALL byla také hodnocena tolerance k zátěži. Metody: Do této studie bylo zařazeno 59 dětí s astmatem (34 dívek a 25 chlapců) ve věku 14,5 ± 1,7 let s diagnózou astma bronchiale v remisi a 162 pacientů s ALL v remisi (87 mužů / 75 žen ve věku 12 - 24 let). Pro porovnání výsledků byly naměřené zdravé kontroly odpovídajícího věku, krevního tlaku a antropometrických parametrů. Všichni respondenti byli rozděleni do několika věkových podskupin, aby byly reflektovány změny kardiovaskulárního systému v závislosti na věku. Pacienti s ALL kromě toho byli rozděleni do skupin podle doby od ukončení léčby. U astmatických respondentů byly měřeny spirometrické parametry (FEV1, FVC, FEV1 / FVC, PEF, MEF). Dále u nich pomocí applanační tonometrie byly měřeny parametry cévní tuhosti (AP, AI, PWV a CAVI) a srdeční funkce (DP / DTMAX, ED, TTI, DTI a SEVR). Pacientům s ALL byla vypočítaná citlivost baroreflexu (BRS, BRSf) a pomocí aplanační tonometrie byly změřené AP, AI, PWV a SEVR. V rámci zátěžové spirometrie a echokardiografie byly měřeny parametry: metabolický ekvivalent (MET), spotřeba kyslíku na vrcholu zátěže (pVO2), velikost levé komory a systolická/diastolická funkce. V rámci statistické analýzy byli srovnáváni pacienti s kontrolama v každé věkové skupině. U astmatiků byly korelovány kardiovaskulární parametry se spirometrickými. Výsledky: Spirometrické parametry se nelišily mezi astmatickými pacienty a kontrolami. Astmatici měli zhoršené CV parametry v porovnání s kontrolami. S roustoucím věkem se u astmatiků zhoršovaly parametry cévní tuhosti a srdeční funkce. Podobný výsledek jsme našli ve skupině pacientů s ALL, kde s rostousím věkem a prodlužující se dobou od ukončení léčby dochází ke zhoršování CV parametrů v porovnání s kontrolami. Závěr: Na základě našich výsledků můžeme tvrdit, že astma bronchiale a léčba antracyklinovými antibiotiky negativně ovlivňují parametry CV systému u dětí a dospívajících. Navíc příznaky poškození CV systému mají vyšší prevalenci s rostoucím věkem respondentů.

Abstract:

Introduction: Atherosclerosis‐related cardiovascular (CV) diseases (CVD) are the number one cause of death globally. Whereas in some cases, CVD is a complication of another primary disease (CVD as a complication of asthma bronchiale is 34.3% and 65% in case of childhood cancer survivors). That is why it is crucial to provide an examination of the CV system in non-cardiovascular diseases. The aim of this work was to collect and evaluate data in asthmatic children and patients with acute lymphoblastic leukemia (ALL) with determination changes in the CV. In ALL patients, tolerance to exercise was also assessed. Methods: 59 asthmatic children (34 girls and 25 boys) 14.5±1.7 years old with the asthma bronchiale diagnosis in remission and 162 childhood cancer survivors (87 males/75 females, age 12 – 24 years) were enrolled in this study. To compare the results, we measured healthy control participants in the same age and the corresponding parameters of blood pressure and anthropometric parameters. All respondents were divided into several age subgroups for reflection age-dependent changes in the cardiovascular system; moreover, cancer survivors were divided according to the time of the treatment termination. Spirometry, applatanion tonometry, and CAVI were measured in asthmatic respondents with estimation spirometry parameters (FEV1, FVC, FEV1/FVC, PEF, MEF), arterial stiffness parameters (AP, AI, PWV and CAVI) and parameters of cardiac function (DP/DTMAX, ED, TTI, DTI and SEVR). In cancer survivors we measured continuous non-invasive beat-to-beat blood pressure measurement, applanation tonometry, and stress ECHO with estimation the following parameters: BRS, BRSf, AP, AI, PWV, SEVR, ET, MET, pVO2 and ECHO parameters of left ventricle size and systolic/diastolic function. For statistical analysis was used program Statistica 13.2 (StatSoft). For comparison different parameters were use the following tests: nonparametric Mann-Whitney test, nonparametric Wilcoxon matched pairs test and Spearman correlation coefficients. Results: Spirometric parameters did not differ between asthmatic patients and controls. Asthmatics had impaired CV parameters compared to controls. With increasing age, vascular stiffness and cardiac function parameters worsened in asthmatics. Similar results were found in the ALL patient group, where with increasing age and longer time after treatment, CV parameters deteriorated compared to controls. Conclusion: Based on our results, we can conclude that asthma and anthracycline antibiotic treatment adversely affect CV system parameters in children and adolescents. Besides, symptoms of CV system damage have a higher prevalence with increasing age of respondents.
